Pregnant after a Loss

s/o cerclage

So I've been reading my WTEWYE and I was thinking ...  How do they dx an IC before it causes m/c?

I'm just nervous bec I'm carrying twins.  How do they check and make sure my cervix is "competent?"  Just by doing a vaginal exam?  If they "catch" that it's dilating in 2nd tri, or whenever, can they do something to stop it/fix it?  I've only heard of cerclage from you ladies who suffered a previous loss due the that Dx.

I know I'm going to worry about everything possible but last time I had the "It wont happen to me" attitude, and well, it did.  So I need to be prepared and advocate for myself.  How do I address this w/my Dr ladies?

  • My dr thinks its what caused me to go into preterm labor at 20 weeks last night and the infection got in there because the cervix was already open.   He has decided to place the stitch at 13 weeks this time to be safe.  He will continue to check it every two weeks until then.  He said the cervix doesn't usually start opening until 17 weeks or so.

     

    I have a friend who went into preterm labor with triplets but this time she is have a single birth and her cervix is fine.

    As far as the cerclage goes, I have no experience with it, but I would just bring it up to your doctor at your next appointment. I tell my doctor all of my concerns and fears, even if it makes me look like a paranoid hypochondriac.

  • Basically, there's no way to diagnose IC until it happens to you.  Especially with multiples, your cervical length should be checked often before 24 weeks by internal ultrasound (dildo cam).  There's no magic good number/bad number, but anything above 4 cm is rockstar.  Anything below 2.5 is short.

    However, those numbers also mean nothing.  It all has to do with the substance that it's made of.  One woman's 2.5 may be the same strength as another's 5.  At 16 weeks with my twins, mine was 2.5 cm and SHOULD have raised major red flags. Unfortunately it did not and I was too naive and unaware to notice.  A week and a half later, I was 2cm dilated, with no cervix, then 10 cm the next morning.  After deliving Jonah (Twin A) I closed back up to 3cm but we knew that it was made of mush and was dynamic, it could change at any moment.

    The worst enemy to the cervix is gravity and pressure, and the only way to see it coming is to monitor for cervical changes.  Supposedly only 1% of losses occur to IC, but I seem to know of too many of those 1% stories..Sad

    Talking about emergency cerclages (placed after 15 weeks or so) the success rate is very low and many OB's don't perform them b/c of the risk of infection.  Usually severe bed rest is prescribed for cervical changes around the viability date.  

    I dont' want to add worry, I just wanted to tell you the facts that I know.
