TTC after 35

Intro and a Question

Hi.  I've been lurking on and off for a bot, and I finally thought I should introduce myself.My DH and I have been ttc (or at least not preventing) since January 2009. I finally went to the RE.  My FSH is on the high end of normal and I have a short luteal phase.  DH's SA is on the low end of normal.

Here's my question: Because I'm 36, RE wants to be aggressive.  He is recommending IUI and injectables and a trigger shot.  He is recommending that over IUI with Clomid.  I feel pretty overwhelmed by this.  It seems pretty extreme to immediately go to injectables, and it seems like a PITA to have to go to the dr. every 2 days, but it would be worth it in the end if it works. I was wondering if you would choose the clomid or the injectables and why. Thanks

  • As someone who's been through 5 IUIs now (including one with Clomid that didn't work because Clomid thinned my lining, something it in fact does to a lot of women), I would rejoice in the fact that you have an RE who is giving you firm advice and wants to be aggressive, and I would go with his plan (especially given the other issues of high FSH, short LP, and DH's count).  Here's hoping it works for you!
    me - 41 (dx: DOR); DH - 53 (no problems); 7/18/09 - married!; 8/4/09 - BFP on first (real)try; 9/14/09 - missed m/c; 9/15/09 - d&c; 11/09 - 3/10 - 4 natural cycles = BFN; 4/10 - dx hyperthyroidism caused by Graves' disease; 6/10 - thyroidectomy; 7/10 - 12/10 - 1 natural and 5 medicated IUI cycles = BFN; 1/11 - new RE; dx low ovarian reserve (AMH .42; 1/26/11 -- BFP (ectopic) from IUI #6; methotrexate 2/10/11; 6/2/11 - IVF #1 = BFN; 9/12/11 - prescreening for DE; 9/15/11 - IUI #7 (unmedicated)= BFN; 11/8 - begin DE cycle (shared risk program); 12/5 - ER (5 eggs/4 mature/3 fertilized/2 left by day 5) 12/10 - ET of one 1BB blast (expanded, "fair" quality), none to freeze; 12/22 - totally shocked by +hpt; beta #1 = 413; #2 = 3952 2/14 - CVS reveals a healthy baby girl! EDD: 8/27/12 DD born 8/31/12, 10 lbs 10 oz and perfect in every way.

  • I can understand your hesitance, but if you've been trying for almost two years, I think you should go with what the dr recommends.  I don't have personal experience with this, but from observing others' experiences, clomid doesn't seem all that effective for a lot of people.  Maybe you and DH should discuss it further with your dr--get his reasoning for why not clomid and why the protocol he's recommending.

  • I haven't been in your shoes yet so I can't really say. I'm sorry its taking so long for you! If you are really wondering and aren't comfortable with the treatment  plan you could always find another RE for a second consultation. Best of luck to you!

  • Hi & welcome!  I went thru alot of IUI's w/ clomid w/ no luck & we were starting 3 cycles of inject & trigger & progesterone supp & IUI's.  Then we'd move on to IVF if no go.  We were truly blessed that the 2nd cycle of the inject & trigger (total IUI #7) worked & we got pregant w/ twins.  Going for all those U/S's & all the shots & that seemed overwhelming at first but now that I look back at it, it really wasn't as bad as I thought & all well worth it.  Now DH & I are hoping that by some miracle we can get pregnant again w/o all that this time around, we'll just have to wait & see.

  • it is overwhelming only at first but I agree with other pp.  Go with the REs recommendation.  Good luck.  We'll be here every step of the way!

  • The only thing I could see making Clomid a better option is if the meds aren't covered by your insurance, since injectables are like 10 times more expensive. They are much more effective though, so as long as injectables are an option financially I would go for it.
