TTC after 35

Possibly switching RE's

So last night I go out for happy hour and someone from work (whom I'm not especially friends with but who knows I'm TTC, as is she) tells me she's pregnant.  This was not especially news I needed to hear right now even though I'm happy for her....but at any rate it led to a conversation about my current clinic (where she also started) and the one she's at now (the other one in town), and she made me rethink my initial resistance to switching.  And as many of you know, I have been so frustrated by the way my clinic does IUIs at 18 hours after the trigger with no real explanation of why when everyone else seems to do them at 36 hours, and when I know I ovulate at 40 hours after the trigger; plus we may be moving on to IVF, and it really does make sense to comparison shop before that kind of decision/investment.  So, today I've scheduled an initial consult with the other clinic for the end of January, by which time we'll know if the IUI I'm waiting on plus the next one (which we've already paid for) has failed and it's time to move on.  I feel good and proactive about this.
me - 41 (dx: DOR); DH - 53 (no problems); 7/18/09 - married!; 8/4/09 - BFP on first (real)try; 9/14/09 - missed m/c; 9/15/09 - d&c; 11/09 - 3/10 - 4 natural cycles = BFN; 4/10 - dx hyperthyroidism caused by Graves' disease; 6/10 - thyroidectomy; 7/10 - 12/10 - 1 natural and 5 medicated IUI cycles = BFN; 1/11 - new RE; dx low ovarian reserve (AMH .42; 1/26/11 -- BFP (ectopic) from IUI #6; methotrexate 2/10/11; 6/2/11 - IVF #1 = BFN; 9/12/11 - prescreening for DE; 9/15/11 - IUI #7 (unmedicated)= BFN; 11/8 - begin DE cycle (shared risk program); 12/5 - ER (5 eggs/4 mature/3 fertilized/2 left by day 5) 12/10 - ET of one 1BB blast (expanded, "fair" quality), none to freeze; 12/22 - totally shocked by +hpt; beta #1 = 413; #2 = 3952 2/14 - CVS reveals a healthy baby girl! EDD: 8/27/12 DD born 8/31/12, 10 lbs 10 oz and perfect in every way.

  • I would switch if you don't think your dr is responding to your questions. Any good doctor would agree that you should get a second opinion or change practices.

  • Well, it's a long story; it's not that the doctor doesn't respond (or that I don't like him, because actually I do), it's that I think the explanation of "we want sperm there before ovulation" based on a fear that I *might* ovulate early (when I have ovulated at the exact same time after the trigger for each of 4 times) is unsatisfactory.  They did once do it according to my timeline, but we had to use DH's frozen sperm, so much lower numbers.  As the colleague I talked to last night put it, my current clinic tends to speak more in terms of statistics than people. There's also the fact that the clinic I might be switching to provides much more information up front on their website -- outlining IVF procedures and meds, for example -- than mine does; it's not that mine won't give me information, it's that the difference speaks of larger differences in terms of approach to patients, if you know what I mean.

    But I need to feel ok with the decision I made to go to my current clinic first -- after all, it will have been 6 months with them, and I did choose them for a bunch of reasons intitially.

  • I think it's completely reasonable.  I actually "comparision" shopped before we started IVF.  I called all the clinics and got their rate sheets - I'm paying out of pocket and it mattered! Plus I had a co-worker who had a bad experience with my clinic and it bothered me.  I ultimately decided to stay but I feel much much better about it after looking around.

  • I agree with the other posts. If you're not happy with your RE, it's OK to switch. In fact, we just made the decision to switch to out-of-state RE (CCRM) after a year with our RE and no success. Plus our RE doesn't have experinece with PGD which we just found out we need because DH and I ar both CF carriers. Also the fact that our RE didn't even test of for genetic issues like CF until the IVF (instead of before the SEVEN IUI's we had!) makes we feel very uncomfortable with him despite the fact that he's a very nice man with a decent success rate for my age range (50% for 35-37). But CCRM's rates are higher and they have more experience with PGD.

     

    I think in this process information is so helpful. There are so many unknowns that our RE's should be giving us whatever info they do have.

  • It can never hurt to get a second opinion.  Go with what feels right for you.  It sounds like they have a better bedside manner including and informing you of all the protocols from the beginning rather than waiting for you to ask.

     Hopefully, you won't have to worry about another consult, but kudos to you for being proactive. :-)
