i don't know what's obvious or not but here's my must haves
co-sleeper (that attaches to the side of the bed)
boppy
vibrating /bouncy chair
lansinoh lanolin nipple cream
we got a big FP high chair last time and got rid of it. too big. this time we're getting the space saver.
needed a breast pump after I had DS. I took people's advice and didn't get it in case I didn't need it, but when I needed it, I was desparate. Ended up having to rent a breast pump until mine came in....found a really good deal online.
Video baby monitor (i liked to watch DS and it helped with my anxiety about his safety)
Baby Bjorn (you basically wear your baby a whole heck of a lot in the beginning)
Swaddle blankets, really cozy soft blanket. You really can't have too many blankets.
Jumper (free standing - not doorway)
Activity mat
Big pack of cloth diapers. I used them to protect the changing pad (so I wasn't constantly washing it), use as burp clothes, clean up messes. They were really handy and I still use them.
Lots of onsies. You'll change their onsie about 4-6x per day so get about 15 so you aren't constantly doing wash. Speaking of wash, put spray n wash on the laundry hamper and spray spit up stains and toss them in the hamper to prevent permanent stains.
GOOD swaddle blankets - these are awesome Aden and Anias. They are really big and they also make great nursing covers and blankets in the warmer months.
I love the boppy for the baby to hang out it, but it really was useless (for me a least) for breastfeeding. I've heard great things about My Breast Friend (or something like that).
I agree with pp, the FP space saver is awesome. We didn't even bother with a regular high chair.
If you do chose to register for bottles, I would get a variety...maybe one of each brand you choose. Until your baby gets here, you won't know what he/she will like. Same goes for pacifiers.
Onsies - plain white onsies. Try and stay away from gerber, they are kind of thin. I like Carter's or Circo (Target) onsies.
Personally, I WOULDN'T buy/register for a breast pump until you are absolutely sure pumping/breastfeeding works for you. With DD1, my milk never came in, she never latched and BFing just didn't work out for us. With DD2, we were successful with BFing, so we ended up buying one. I highly recommend the Medela Pump In Style Advance, if you do decide to buy one ahead of time.
That is all I can think of now.

A white noise/music box (we have a HomeMedic sound spa with lights that we still use)
Yes! The Baby Crack Machine! DS still loves his and runs into his room to play with it.
A video monitor. My sister told me I wouldn't need one, but once we started putting DS in his room for naps and nighttime, I HAD to be able to look at him.
A GOOD carrier: my favorite is the Ergo
Muslin swaddlers: Target used to have the Aiden and Anais ones. It will be summer, so lightweight will be necessary, plus I found if I draped one over the carseat while DS was sleeping, he would sleep longer. They also are easy to stuff into your diaper bag, because they are thin. Oh, and my LC showed me how to tie it around DS and I, so I could sit and breastfeed him handsfree. SO many uses.
Oh and I heard SO many great things about "the Ultimate Crib Sheet" and how they are a "must-have" that I purchased two of them. I have yet to use them. Maybe they will come in handy when DS is potty-training.
These flannel multi-use pads came in SO handy, though. I would put one down on the co-sleeper, or in our bed since DS slept with us a lot. He was constantly leaking through his diaper and this saved our sheets (and the co-sleeper's sheets). Once he fit into overnight diapers, we stopped having the leaking issue.
