This year DD is giving her grandparents, aunts & uncles a 2011 calendar I made on shutterfly. I know I am biased, but it turned out so stinkin' cute I cannot wait until they see it!
On DH's side, the understanding is that everyone only buys gifts for the kids (between the 4 siblings there are only 3 kids, 2 of them DH's, so it isn't too much). His parents are both deceased or we would get gifts for them. On my side, we get gifts for my parents and teh kids but not siblings unless they are physically present on Christmas.
ETA: I generally do all of the gift shopping.

We don't because SO isn't big into Chrismas. He thinks presents should only go to kids and all of his nieces/nephews are grown. We only buy for his mom and his grandma because I insist on it. Usually, we shop together. I make a few suggestions and then he usually picks one.
In fact, we wouldn't even exchange presents if it was up to him. I'm letting him win on that concept this year (due to budget), but will fight again next year - lol.
We buy for my parents, his brother, SIL and their 3 adult kids, my brother's kids, his aunt and cousin, my BFF, and his two BF, one BF wife and three kids (Michael was named after these two men and they are extended family).

Besides my MIL & FIL we buy gifts for his brother, sister & nephew. DH's dad's side of the family also does a draw so we have 2 other presents to buy. He picks out his dad's present, but I usually have input on the others and we buy them together.
We don't exchange presents with my side (which is only actually my dad and his wife). They get the kids something and this year we are getting Nancy's girls a little something.

DH's extended family is insanely huge (each of his parents have like 11 siblings EACH) so they all keep to themselves and do a huge family reunion every other year. That said, it's just MIL, FIL, BIL. I have enough of my plate with my family so I leave his family up to him... though this year I really need to hop on getting some pics printed (at the least) of their first grandbaby.
Our family is tiny!! DH buys for his parents and I buy for mine. We do a Secret Santa with our close friends in town so two gifts there but usually those are really easy because we know what the friends need/want.
I am getting the grandparents calendars of Matt and for my parents I am donating money to their charity. I have no clue what DH is getting for his parents.
