I think they run a little big. I have a skinny average height 4 year old. 4T is a tad small usually, 5T or Boy's 4 perfect, and Boy's small or 5/6 a bit large now. We just bought him the 5/6 at Children's place, and it's not just a bit large, but huge! He can wear the jamies, but the pants look a bit silly.
If you want it to fit well, I'd go with the 4. You could probably use a 5/6 now for a sweatshirt of jamies, but it might look funny for a nice shirt or sweater.

They run small on my tall, thin DD and about true to other sizes on my DS (chunky monkey). DD wears 3T everything and 4T shirts from TCP and 3T pants...so the shirts are a bit small I think. Pants are true to size I would say though.
Their 12m size is 18-22 lbs, which is more like 9m in Carter's (16.5 - 20.5 lbs). DD wears 18 months in CP, 6-12 months in Gymboree and Gap, and 12 months in Carter's. I don't know how their big kid sizes fit - clearly we haven't made it out of the baby clothes yet.
