Triggered, Thrown for a Loop at appt, & IUI help?!

Okay, I'm going to try to make this as short as possible!  I'm CD13 now, took 5mg Femara/Letrozole days 5-9, started surging last night so I went in today for my first follie check and post-coital test.  Follie check showed one follie at 19mm!  Perfect!  They gave me my HCG trigger shot in the office and we had planned on doing TI since all of DH's numbers have been fine. 

While doing the PCT, she could only find 5 sperm alive... this was approximately 4 hours after we had sex, so right in the middle of the window they want.  She said this could be a sign of having an anti-sperm antibody (think that's what she said) or hostile mucus?  Either way she said it wasn't good and didn't think TI would be worthwhile.

She encouraged me to consider doing an IUI this cycle.  We won't be covered for IUI's until our new insurance kicks in on January 1st, but it's only $375 OOP.  I talked to DH and we both agreed to go ahead with IUI tomorrow morning and pay the $375 so we don't waste this cycle and the meds we've already used if TI isn't going to do any good. 

I was just thrown for a loop with this new information and seemingly jumping into IUI tomorrow with no real knowledge of how that all goes down.  Can someone give me the quick & dirty on the IUI procedure?  Will I automatically go on progesterone or do they want until the 7dpo check to start that?  When do you typically have an IUI after trigger?  I'm not sure if our timing will work or if they're pushing me to fit into their schedule last minute?

  • Wow, what a whirlwind!

    The way they do IUI at my RE is that DH goes in and makes his contribution, they count and wash, and then inject about 2 hours later.

    It's super simple, it's just a small catheter that they use to push the sperm past the cervix.  It's uncomfortable but not at all painful (or at least not in my experience).

  • That is EXACTLY what happened with my first IUI!

    I believe you will start progesterone without the b/w 7dpo...at least that's what I did and since it is the same clinic...and my P4 had never been low or anything, I think it is just protocol.

    We typically do the IUI 36 hours after trigger...but my first one I did just a little over 24 hours after trigger.

    The procedure is very quick and easy. Your husband will give his sample and then they'll wash it and all. Once it is ready they put you in a room, you undress from the waist down, they come in, put the catherdar in, inject the sperm (after making sure it is your husband's) and then you just lay there for 15 minutes. Feels a lot like a PAP.

    Our clinic typically does a collapse u/s after the IUI and that's kind of neat to see.

  • Hey there cycle buddy!! No worries with the actual IUI, easy as pie!

    What usually happens is you go in, get your legs up there in the stirrups and in goes the speculum. The nurse puts a long flexable cathetar into your ute and injects the sperm. The last IUI I had, when the cathetar went into my cervix, there as a little pain but not much. The whole thing takes o 2 mins maybe. I went on Prog. supps 3 days after IUI last cycle becasue the Clomid was making my lining thin. So im not sure about checking 7dpo, its worth while to ask though. I have always had my trigger 24 hours after trigger, if not later.

  • Boy does his thing in a cup and they wash it.

    A few hours later girl gets into the stirrups and they shoot the swimmers in. It's basically like a pap. After that, no swimming, baths, or other submerging.

    My office does p supps for injectable IUIs or anyone with a short LP.

    36 hours after trigger.

  • I have had 6 IUI's so I can help you with that! Basically it's like a pap, however they insert a catheter into your uterus and then shoot the sperm through the cath so that the spermies don't have to do any swimming on their own. It's very quick and can be mildly uncomfortable, but bearable.

  • hmm, sounds like a quite a whirlwind huh? Let me see if I can help at least a little bit. Ok well I've had two IUI's and they were 24 hours after I triggered. Some places do back to back IUI's and will do one the day after trigger, and then 2 days after trigger.

    Progesterone is usually only for after IVF, unless you have a known low progesterone issue? In which case I'd imagine they'd start you on some form of progesterone a day or two after IUI. The actual procedure is super simple...they just put a speculum in, insert a catheter that contains the washed sperm...in they go...lay there for 15 min(some RE's may require longer).

    HCG trigger 36 hours before IUI

    The IUI itself was quick and painless.  Since this is your first IUI they may do a collapse u/s to make sure your follie released the egg.  Make sure your bladder is full.  The RE will insert the catheder and then the washed sample.  After the IUI they will have you lie on the table for about 10 minutes.

    My RE starts all of his patients on progesterone after all procedures.  They will do b/w 7 dpo and make adjustments if needed.

    Side note - I didn't know this until the day of, but you have the option to go down the hall to help YH give his sample.  I was completely shocked and embarrassed Embarrassed

    Madelyn, that's too funny that the same thing happened to you.  Did you end up having the anti-sperm antibody?  They said they'll draw it with my b/w next week.  When do they typically do the collapse u/s?

    Did you go to ART?  I just moved there from UAB since my insurance was going to be changing in January and I'd have to move then anyway.  Thanks for the info on how everything went for you... I had my HCG trigger today at about 10am and will go in at 9am for the IUI so just about 24 hours later... not the 36 hours I'm seeing a lot of girls saying they did.  And yes, I did know that about the SA and IUI sample collection... DH wanted no part of that and I didn't either!  lol

    Can someone give me the quick & dirty on the IUI procedure?   you lay there in stirrups, dr inserts catheter into cervix and then squeezes in the gizz

    Will I automatically go on progesterone or do they want until the 7dpo check to start that?  my clinic automatically puts you on progesterone, crinone for IUI

    When do you typically have an IUI after trigger?  36 hours

    I'm not sure if our timing will work or if they're pushing me to fit into their schedule last minute?   can you ask for it to be 36 hours after trigger, it's all about timing

    It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

    No, not anti-sperm antibody, just no cervical mucus at all. I've never had any. When people would talk about their CM I honestly had absolutely no idea what they were talking about. The only time I've ever had CM or EWCM has been on inject cycles.

    Ohhh, okay.  I've never had much CM and have never seen EWCM, but she didn't say anything about me not having any... just that what was there must be killing them since DH has always had great numbers on his SA's.  Since we had sex this morning before the PCT and we're having the IUI early tomorrow (7:30 sample, 9:00 IUI), I'm thinking we shouldn't do anything tonight... right?  Just wait until tomorrow night?  I'm sure they'll tell me tomorrow but just curious what your experience has been.

  • I think the PPs have covered everything but my RE says to come in with a full bladder for your IUI.  I forgot with my last IUI and they had a hard time getting the catheder to go in.

    Yep, ART with Dr. Long.  If it were up to me and MH, Dr. Long would deliver the babies.  I loved everyone at that office. 

    24 hours is fine since you started picking up your surge last night.  The collapse u/s will confirm that the egg has been released or that it's about to be released. 

    The RE will be able to give you the post-wash count tomorrow during the IUI.  The other info - morph and motility - will likely not be available until the next day.  A nurse called and talked to me for 10 minutes or so about the report.

    Yea, I wouldn't have sex tonight. Wait for the IUI and then have sex tomorrow night and probably the next morning.
