I'd ask your new OB if they have a release of information form you can give to your old OB. They should be able to get the report over there from your file. Other option is go directly to the records people at the hospital you gave birth at and request it from there.

My new OB said he'd like to see it, but that it was not 100% necessary.
I contacted the records department of the hospital I delivered at. They mailed me the operative report and I handed it to my OB. They actually had a records request form online that I printed out, checked a box that said operative report, gave my name and the date of birth and just faxed it in. Very simple.
My new midwife said she needs to see the scar report (I assume because the type of incision and stitching matter). I think I am going to order the whole record though because I want to understand what happened from the OB's perspective.
