I've read a lot of things that say it's a good idea to have, besides your spouse, a woman who's already given birth in the delivery room with you especially for the first baby. My mom is not an option and although I dearly love my MIL, she drives me a bit crazy so that's not the best choice either. We haven't looked into a doula but perhaps that's an idea.
For those of you who don't have moms or older sisters, who did you/do you intend to have in the room with you?

there's no need to have someone else in the room imo.
it ws just the husband, myself, nurses & the OB when i had my first.

No one. My hospital's policy is you need the doctor's consent for extra support if you want more than 1 adult in the room with you.
I'd definitely recommend a Doula! I didn't want female family members in there because they'd drive me nuts. So, I asked an old co-worker (who's also a Doula), if she'd be willing to be there. We were close, so of course she said yes! She's had 8 kids of her own, and has been a Doula to over 70 births. If you can't find or don't want a Doula, maybe look to a friend who has had children already. HTH
During the 1st part of my pregnancy with DS I had said I didn't even want DH in the delivery room. Now I'm glad he was in there, along with my Dr. & 1 nurse, but that's all I feel the need to have in there again. If I had a doula I'd obviously have her in there, but I don't, so I won't. That's all.
I only wanted my DH with me and the delivery went fine. DH will be the only one with me this time too.
In addition to the nurses and Doc (and anesthesiologist if necessary)...DH and my Mom. Only. FIRM.
NO MIL.
NO interns.
NO friends.
NO random passersby.
Thanks for all the fantastic replies. We've had 2 out of 3 birthing classes at this point and from what I've heard there and here, it'll be best for us if it's just DH in the room with me. Best of luck to all the MTBs!
