I usually build the cake right on some cellophane so it didn't necessarily need a 'base' since once I pulled up the cellophane it was secure. I did however use rubber bands to keep the diapers rolled...but I covered up the bands with ribbons to decorate the 'cake.' And yes, all diapers were usable. I put a variety of 1 and 2 size diapers. Very little newborn diapers.
Just had my shower this weekend and my sister made a super cute diaper cake. She just used a cardboard bottom (wrapped with something) and then she put a bottle of Pedialite and wrapped diapers around it. If the link works, take a peek. She found the instructions online somewhere.

yes, you can use them. as long as they were carefully rolled and arranged. but use your better judgement-check if some got dirty.
for the base, i usually use one of those round cardboard cake bases, or you can use a simple round tray.
