We have given him a crocheted blanket on cold nights in the past weeks. We have the Angelcare monitor, and he's strong and moves around a ton, so that's how I keep myself from worrying.

DS has had a blanket since about 2 mos old. We keep the bedroom downright freezing during the summer and the nights got cold quick here so he's been covered for a while now. He hates sleep sacks!

We're using sleepsacks right now. I won't give a blanket until he's a year. I'm a nerdy rule follower like that! I could never forgive myself if I gave him one and something happened!
I let DS have a crochet blanket (with large holes) once he started yanking things off his face. If anything covers his face he immediately pulls it off. I think he started doing it a month ago.
Aidan uses a blanket- has since 4 months. We use the angel care monitor too so I am not as worried. He can crawl and stand on his own so I feel like he is strong.
I think I gave him a holey, crocheted one to use at about 5 months. Since about 6 months (when it started to get cold) we have given him a fleece one to keep him warm. He never puts it over his face...in fact I'm always sneaking in there after he's asleep to put it back over his body because he wiggles around so much when he's going to sleep that it comes off. I'm personally not worried about him having a blanket in his crib, now that he can lift his head up and roll around like crazy.
DS has managed to get himself quickly tangled in blankets while playing on the floor, so I'm too paranoid to put one in the crib just yet. I put him in fleece pajamas and keep the house a comfortable temperature.

This but I think it was around 3 or 4 months that we starting using them - but just on nights that I thought he was cold in the a/c - and we put it below his waist. He was sleeping right next to me at the time too. When he moved to his crib at about 6 months - we put warm blankets on him since the nights get cool. He moves all over the place in his crib so I am constantly recovering him too.
My DS also always yanks things off including his bibs and was playing peek-a-boo with everything since he was 3 - 3 1/2 mos old. However, it has only been just recent because of the cold nights that I have given him a crocheted blanket at night - by morning it's off though. I was crazed at first, but my DH is too friggin lazy to put him in a sleep sack before he falls asleep ( RRRRrrrr). Bedtime is his thing though, since he works all day. The first night I had no real option other than to blanket him or pick him up and put him in my toasty bed with me... :-D I was up all night watching though... He's been fine with it, but he's a BIG BOY with his own mind of doing things already. Follow your gut since you know your baby and abilities.... GL
I won't be letting my LO sleep with a loose blanket until he's at least 1 year old. He sleeps in Halo Sleepsacks. DS1 slept in them until he was over 2, and that's my plan with DS2. Loose blankets get kicked off and don't keep him warm, and just aren't worth the SIDS or suffocation risk.
