Let me start by saying I have been seeing an OB and a Peri since the begining of my pregnancy and they seem to never be on the same page.  I failed my 1hr gtt at 12 weeks (level was 144) and then "technically", according to OB passed the 3hr.  She said at the 3 hr I did have one slightly elevated level but the other two were perfect so it wasn't enough for a diagnosis.  Now, I went to peri about 2 weeks ago and she said I needed to be taking my BS 4x/day, when I mentioned I hadn't been told I failed anything to that extent she replied "well I don't expect you to since you take Metformin".  She said that my BS 2 hours after meals should be UNDER 120... well that hardly happens.  Usually its in the 120's 130's and randomly it will be in the 140's.  I am kind of getting freaked out because peri mentioned that at 30 weeks I have to come in twice a week for fetal monitoring b/c of the rise for STILLBIRTH?!?  I am really getting freaked out that my BS has been over 120 quite a bit.  I will admit that I don't eat very healthy but I am vowing to start! 

Any advice, tips??  My OB seemed so unconcerned about it all and now my Peri seems all worried!  What does your OB/peri say your BS should be??  What kind of diet do you follow?  Do you think mine have been consistently high enough to cause any issues??

  • My fasting sugar has to be under 95 and 2 hrs after meals I have to be under 120. I take glyburide, so I have NSTs and BPPs once a week. My diet is a carb controlled diet, 15 carbs at breakfast, 45 at lunch, 60 at dinner and 15-30 at snack times.You should talk to your peri about going to see a diabetes counselor or nutritionist so that they could help you out with a diet.

    Most women don't seem to get diagnosed with GD until around 30 weeks and have blood sugars more out of control than what yours have been. Don't stress about it too much.

  • Same as the PP--my fasting sugar needs to be under 95 and under 120 two hours after meals.  My meals are set up as: 30g carbs and one ounce of protein at breakfast, morning snack, afternoon snack and evening snack.  I need to have 3 ounces of protein and 45-60 grams of carbs at lunch and dinner.  The class with the dietician was really helpful.  Maybe see if you can attend one of those to help you with meal planning.  I have only been following the diet for less than a week, but for the past three days my BG numbers have all been in the right range.  My doctor said I will have some more ultrasounds to check for size.  Guess that is the silver lining in this--getting to see the little one again!  Also, once I hit 34 weeks I will have non-stress tests too.

  • Oh and the dietician also said that exercise can really help with lowering blood sugar--so I have started trying to walk to help.  She suggested walking 3-4 times a week and nothing strenuous-just get moving.  She said it is best to walk an hour or so after eating.

  • Not GD, but Type 1(I know it's different) but my fasting sugars are supposed to be under 90 and 2 hours after eating they are supposed to be 120.  I would watch my carb intake, but I wouldn't stress too much about your numbers.

    And my first visit with peri, the first thing he told me was You do know you have increased risk of stillbirth and increased risk of down syndrome(because I'm 35).  Yes, thanks so much for focusing on the positive.

    This is what I was going to say. And I also have to say that I think I'm getting screwed!  I don't have to check my fasting anymore, but I have to be under 120 ONE hour after meals.  Holy crap, I wouldn't be considered diabetic if I got to be under 120 at two hours!  I agree that you should see a dietitian.  They are very helpful.  Make sure you see one that is specifically a diabetic dietitian though, most likely at an endocrinologists office, because they will give you a more accurate diet.  I saw one at the hospital and one at the endo and they were completely different.  The one at the endo was much more informative and very helpful.  

  • I was diagnosed pretty much the same time as you were. The only difference being that my OB was very quick to take me off metformin and put me on insulin. I admit, I resisted it for a while but by week 15 I gave in to the insulin god. 

    I went for Diabetes education class where I learned about carb control but the diets were very very hard for me since ever since I got pregnant I just dont have a pallet for meats of any kinds.... so there goes protein out of the window. I have really just been eating what I feel like but still monitoring my BS very very closely.

    Per my OB, I have to be under 90 for fasting and under 140 one hour after meals. Initially, there were a lot of fluctuations in my insulin dosage but I have stayed pretty consistent for the last 4-5 weeks. I do every now and then splurge on my carb intake and do land getting reading upto 180-200. But its usually just once every 2 weeks or so. And I let my OB know of my splurges. She thinks its okay to cheat once in a while.. so I am good.

    There is definitely better control with insulin than metformin.
