We do one every year for the grandparents (they LOVE them!), and we do pictures from the last year. For example, on January in the 2011 calendar there are pictures from January 2010. Last year the whole calendar continued that way. This year I already ordered them (because I got a great deal!), so November and December just have other pictures from this Fall.
I think either way is fine if you have never done one before though.

My sister and I both have babies about the same age, so we gave our parents one last year for Christmas when the kids were 8-9 months old. We used pictures from birth - that Christmas. For this year we'll use pics from the past year (i.e. Xmas 09 - Xmas 10)

I am doing ones this year of all the grandkids - we are using a bit of everything, but generally pretty heavy on the last few months.
That said, I would use whichever pictures you like best
Regardless, by the end of the next year they will be "old"!
