Questions to ask daycare centers?

My DH and I are starting to look into daycare centers for the bambino.  Does anyone have a good list or know where to find one of questions to ask the daycare centers? TIA!

    Here were some of mine - - I'm sure there are others you can ask too.

    1. What do we need to bring? (Diapers? Formula/Breast Milk? Wipes? Crib sheets? Blankets? vs. what is provided?)

    2. What are the hours of operation? How much if late for pick up?

    3. Are the teachers certified?

    4. What is your star rating?

    5. What is the ratio of babes to teachers?

    6. What age groups are the rooms and when/how will your kid be able or ready to move up?

    That's a start - - I'm sure others will have other questions as well. GL! Finding a daycare can be tough.
