For us - No, it does not go back in the fridge because you can not reheat it a second time. Once the bottle was used, we had a rule to finish it in 4 hours although I'm not sure where that number came from.

Yep you can offer it at the next feeding and then it should be thrown away.

This. We would not refrigerate it but would leave it out and use it within 4 hours or toss it.
