We just started last Friday after her 12 month appointment. The Dr. said I can go straight from formula to milk or I can go easier and mix it 50/50.
I didn't want it to be too much of a shock on her system so I did the 50/50 mixture. She didn't seem to notice a difference. The next day I went all milk and I think she likes it more than formula because I used to struggle with her drinking more than 4oz of formula at a time. Now she's drinking 6oz of milk.
Katie hasn't had any issues with cow's milk so far.

We began the switch at 11 months and are proceeding like this:
11 months - replace afternoon bottle with snack and milk sippy
11 months, 2 wks - replace mid morning bottle with snack and milk sippy
11 months, 3 wks - eliminate morning bottle and move breakfast time up
12 months - eliminate bedtime bottle and replace with story time
We did the same thing with DS and it worked and is working great. We were able to eliminate bottles and formula at the same time.
