Hi All. How do you keep track of your child's immunization records? I feel like a loser for asking. Am I supposed to ask my pediatrician for a book? I remember my mom had a book for me. Do pediatricians still do that?
My doctor does a printout from the computer, which is what I've always provided the girls' preschool. If you ask the nurse during your next visit, I'm sure they'll give you a copy of the updated record.
I have the exact same form for both girls -- received from totally different pediatricians in different cities -- the state of Texas vaccine form. I received it on our first visits with the pedi.
We did this so we don't have to keep providing the daycare updated records. Schools can access the database. Because my hubby is a firm believer in keeping files for everything, I also have a hard copy document that I received from the hospital.
Dx: PCOS and short luteal phase
18 cycles (3 with our RE) - Metformin + Clomid + HCG booster did the trick!

We were asked at the hospital if we wanted to register ds for the Texas immunization database. This way if you ever move they are always accessible.
