questions for new DCP?

Does anyone have a list handy to ask potential DCPs? I just got a job after being home with DS for 7 months, and I am having crazy anxiety about finding the perfect childcare for him. I know i won't ask the right questions and will forget something since I am so nervous about the whole thing. Does anyone have a list or a link handy that may help me with this? TIA!

    looking a little at both, but thinking of in home due to more flexibility with our weird schedules.

    I'm a SAHM but my nephew is in IHDC and here are a few things she had to work with--

    Is it ok for LO to ride with them if they need to run errands?  

    Is it ok for them to stay with anyone else in the home?  Ex--if you won't get off until late and she needs to run to the pharmacy before they close, is it ok for LO to stay with her teenage daughter for 20 minutes?) 

    What other age kids will she be keeping? 

    Are there pets in the home and will they be around LO?
