Gonal-f Compassionate Care program - Has anyone applied?

I was wondering if anyone else has applied for this med program?  I have the app and am getting all my info put together for it, but it doesn't say what the income requirements are.  I am wondering if I am wasting my time applying for the program because we make too much money.

    i used compassionate care - and i am so grateful there's a program like that out there.  it didn't list income requirements, but i can tell you that we qualified with a combined income of $70K.  we had zero IF coverage.  it provided gonal-f, ganirelix, and ovidrel at the time.  the only thing i had to pay OOP for was ordering two additional days worth of ganirelix.

  • Thank you for your reply.  The program still includes all those meds, but they said their are income requirements and we have to turn in our pay stubs and 2009 tax forms.  They said they will take into consideration the OOP expenses we have already paid this year in addition to the costs we are going to incur when looking at the income requirements. 

    How long after you filled out the app did they get back to you about whether you qualified?  I start BCPs as soon as AF shows up so I am hoping I can get everything together in time.  I am hoping and praying we qualify because it will help us so much.

  • I used the Compassionate Care program as well & they were super fast!! I applied & was approved the same day. It really depends on how fast your insurance company gets back with them etc. The lady that helped me said that it wouldn't hurt to write a letter explaining what all you have gone through etc. Then they will send your RE like an order form & they will ship it directly to them. I think that I applied on a Friday & I had my drugs Wednesday if I am not mistaken.

  • I just filled out my app in late July. I was afraid of the income requirements too, but I heard that it was VERY high. I actually called to ask, and she said that everyone is concerned about that, but it's rarely and issue.

    I submitted everything (don't forget your birth certificate and all your OOP reciepts, like I did the first time) and once they had everything, she called me within 48 hours. Your clinic does have to fill out some paperwork, but as long as they are quick with that, your meds should be delived within a week. They are really fast!

    Kuleama I actually have to go down to the County Recorder's office to pick it up this afternoon.  For some reason I can't find the copy I have.  Does it have to be an original copy or will a photocopy work?  My clinic just sent me over an accounting of my expenses so I am good to go on that end.

  • I'm just now applying for it too, in fact I got my big package in the mail today with the materials and some other stuff.  I was worried about the income requirement too but after reading all these responses, we should be good.

    My only concern is my RE suggested doing this but wants me to try Femara first and you can only apply one time, so I just wonder if I should hold off on the paperwork and see how I react to Femara IF AF ever frickin' shows up.

    I was wondering about this too.... 

    I think that we may qualify for compassionate care also.  I'm having a tough time with their website.  It says I have to register to get an application, but when I try it seems like the registration is designed for someone associated with the RE's office.  Does my RE have to register for me to get the app?  We're trying to do this ASAP, so any info would be incredibly helpful.

    Alternatively, if anyone has a PDF of the app that they could email me, that would be amazing.
