I know they can be useless, but there are certain things I want/don't want (no hep shot, no bottles, for hubby to be with baby if I have a csection, etc). Did anyone write one... Would you mind if I took a peak at it? I'm not sure *how* I should write it out.

Here's mine... I submitted it to my ob/gyn probably in my 8th month and discussed it with her. I took copies of it to the hospital and gave to the nurse when I was admitted.

I?d like to have a natural birth with minimal interventions and no pain medications unless I request them. I?d prefer not to have an IV and to be able to stay hydrated by drinking clear fluids. I?d prefer to not have constant fetal monitoring unless the baby is in distress.
To manage pain, I?d like to use techniques such as walking and changing positions frequently. I would prefer to only have essential people in the delivery room. My husband and sister, sister's name, will be there, but no other family members or friends. We?d prefer not to have medical students or residents present during the birth.
I would like to have a mirror available so that I can see the birth and also to touch my baby?s head as it crowns. My husband would like to cut his cord. My husband will accompany our baby during any testing and evaluation.
I would like to breastfeed exclusively and as soon as possible. Please do not give our baby a bottle or pacifier.
I would also like my son circumcised at the hospital.

I had one but I didn't actually bring it to the hospital with me. The things I was set on were few and easy to communicate. For example, I did not want the nursery to give Liev sugar water, a paci or a bottle. My husband accompanied the nurse the nursery when they took Liev up and just let them know. They honored my wishes.
I did not want my arms tied during the c-section so I told the anesthesiologist. He did not budge, said it was hospital policy, etc. so whatever. They tied my arms down and it was all good.
My plan was to go in to the hospital with an inside baby and leave with an outside baby.
Childbirth can be soooo unpredictable that I decided I'd go with the flow.
@ Baptist they have you sign a form for the Hep B shot so you can opt. out of you want. They also have Daddy accompany baby to the nursery. Most things were communicated with the nurses before hand and in the event that anything happened with me DH knew my very short list of request and he could communicate them.

I know things can be totally unpredictable, but I'm having issues letting go of the control factor :x I think since it's #1 I'm aiming for that "perfect" birth, KWIM? As long as we both end up healthy and happy at the end, that's fine - but I'd still like to have that plan there just incase I forget to mention something in the heat of the moment!

But if you have certain requests, it can't hurt to write them down so you don't forget them! All I had to do was let the doctor and nurses know if I had a particular, feasible request. Always go in with an open mind, child birth is extremely unpredictable and if you go in with your mind 100% set on something, you are bound to be dissapointed.
