i don't have a specific rec. but my dd loves any type of doll. she's got so many and loves to mommy them all. one of her first baby's was a soft doll. i think soft would be good for a 4 month old
Marion got this doll not too long ago and when she comes to the hospital to see baby sister she's going to get a little take care of your doll set of accessories.
eta: because it's the bath doll, it's all plastic, which I love.

That will be nice if she gets sick a vomits on it. There is not much worse that a puke covered lovie.
I really like the Pottery Barn Kids dolls. DD doesn't have one yet, but I'm thinking that Santa might bring one this year. The reason I like them is that they're all fabric. DD loves to take her dolls to bed with her. There's one big plastic arm/leg/head one that she insists upon taking sometimes and inevitably every time we have to go in there some time during the night to soothe her because she rolled over in it and it hurt her.
DD has Chloe from PBK. She's well made, and her legs have wires in them so she can sit up but she's still soft enough to sleep with. Unfortunately, her pants will NOT stay up, so Chloe is always mooning everyone... or is naked. Her shoes won't stay on either. DD likes the doll, it's not her favorite, but she is playing with it a bit more these days. I think it's not her favorite because it's not a baby, it looks like a big girl.
This fisher price doll was her fave for a long time. But her clothes are her body, and after a couple washes, she's looking pretty pilly. The hat is impossible to keep on so just throw it away and save yourself some trouble.
The other doll she likes a lot is so cheap that I can't even find it online. I got it at BRU, it's a potty training doll and came with a bottle and a little plastic potty and it pees when you pour water in its mouth. I want to say it was maybe $15 or less. I got it because she wanted a boy doll like her baby brother. It is anatomically correct. It came in a little blue kimono onesie, but she's kept him naked from day one.
Re: Baby Dolls
Her daughter is almost 3
The target circo dolls are nice and rather inexpensive. The body is cloth and legs, head and arms are plastic.

This doll was a huge hit when it was received by my 2yo as a gift. Her aunt gave it to her and she loved it.
So Tasty, So Yummy
this. Bella loves her Cabbage Patch Doll.

Charlotte also has the bath baby doll from Target (Circo) with the accessories, and she loves it.
She has been given two different Cabbage Patch babies, and loves both of them as well.
