Ok so I go on leave in 20 days or so.
My HR dept says my Dr office completes the state disability forms, my Drs office says my HR dept completes the forms.
So I went on the edd website myself and printed out a few I thought were the correct ones and am bringing them in to the Dr this Friday.
I was able to DL the DE 2501 form, but then the other form had to be ordered... does that sound right? :
ANYONE have some insite on this?

Agreed. Totally confusing.
My HR was completely different because we have self-disability insurance.
Any chance you have Kaiser? I know they have business offices at some locations to help people with paperwork.
I practically left my meeting with HR in tears because they made it so dang confusing!
for me, the doctor gave me the forms. I filled them out and then gave them back to the doctor because there's a section on there where the doctor has to confirm your pregnancy, etc. My doctor's office sent them in for me.
ETA: I pretty much run our HR department, and out of 4 pregnancies or so in our office in the last couple of years, I have NEVER seen the pregnancy disability forms or had to fill them out for anyone.
I agree though it is TOTALLY confusing!

Ok I figured I would skip stopping by my HR Dept. I'm w/PAMF...
They do have a disbility dept so I'm hoping that me bringing in the forms will get something moving...
that's really odd - I'm with PAMF (mtn view) and they took care of everything - I just dropped off the form (with my portions completed) to the reception staff at the ob/gyn desk and they took care of everything else.
