When did you decide to do it?
I work at the hospital I will be delivering at, litterally downstairs from the birth center. I work in an office where it can get stressfull, but nothing too, too bad, and it's not physically demanding (I answer phones, greet patients and enter orders, etc.). I originally planned to work up to my due date but am now starting to contemplate taking some time off before the baby gets here, maybe the week or so before. How soon should I make the decision? DH and my friend at work both say to just wait and see how I feel as I go. But should I at least let my boss and office manager know I'm thinking about it? If so, when should I do that?

It all depends on your relationship with your boss, and how it works with your FMLA. If it would eat in to your paid days, I would try to wait and see. If that does not make a difference, I would see how you are feeling around 36 weeks to see if you can make it to the end, or if you need a break.
I worked until 28 weeks. I took care of a 14 toddlers and it became too demanding. NJ has disability so when I went out, I claimed it so it all worked out.
My friend said she was going to work until the end but left 4 weeks early.
I am still working everyday and I commute an hour both ways to work (more if traffic on the way home). Thankfully I am still feeling pretty good. MA is only 6 weeks leave for a vaginal birth, 8 weeks for c-section and I plan on taking 12 weeks so I am saving my vacation time for after the baby is born.
I think it is hard to plan "taking a week off before the baby" because you really don't know when you are going to go into labor. My OB thought I wouldn't make my due date but every day it is getting closer and keep it in mind it is only a guideline - you could be two weeks before it or after it.
If I was feeling miserable it might be a different story but I could do some work from home if I needed to.
