Sorry ladies I haven't been on much, work has been busy and I just needed a mini break of bumping.
Sat down with the RE to discuss the next steps since Clomid was a failure. I have confirmation that I have a form of PCOS on the ovaries and high Androgen (sp?) levels which is giving me the enlarged ovaries and also part of the reason I don't ovulate.
He told me what medication I would take for injectables and how much. 14 Gonal-F
I had no clue you need that much, Lupron and Ovidrelle. He broke down the percentage of pregnancies and ovulation for the drugs at my age. Then he spoke about IVF and explained that it may cost more up front, but there is a better percentage rate with IVF instead of just injectables, plus I have to do the medication once if I produce enough eggs. Doing the meds for IVF would produce roughly 6 cycles worth of embies if all goes well. So he said IVF is best or I can do injectables for a round and see.
He also said he was willing to try Metformin and high dosage of Clomid for one round to see if my ovaries respond to that. He said it is cheaper than my other options and he would be okay to try it once.
I asked if my weight was also causing ovulation issues, he said yes it very well could be. If I lowered my weight by 70, 80 or even 100 lbs he said it would lower my androgen levels way down, which would allow my body to produce more female hormones in order to ovulate.
So I will try metformin and clomid in August. If it does not work, I have requested to be placed on BCPs and work on losing weight. He said going on BCP will help the androgen levels which can hold me back from losing weight if I went without. I need to lose a lot of weight anyway and I haven't been motivated until now. It's not a wise choice to do IVF when I could have saved a lot of money, made myself healthier and maybe trigger my ovulation just by losing weight.

I don't know what to say. You can drop the weight that is a non issue, yes it will be hard, but you can do it because the prize might be a baby! I wish you all the best.

Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)
IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN
IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.
IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132. Lil is here!
TTC#2: Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.
IVF #4: BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN
IVF #5: MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN
IVF #6: (New RE): Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN

I think he has a good plan. I had great success with Metformin and the South Beach/Weight Watchers diet. It seems to have helped the length of my cycles (made them 28/29 days instead of 35+). I have not charted, but I did have positive OPKs. And above all, I am feeling so much better!
You can do it. It will be hard, but once you see those pounds coming off you'll be even more motivated.

Sounds like you have a few options and options are always good! I too am almost in the same boat, but in the beginning stages. I am considered "overweight" but dont consider myself obese. I am heavy....but according to the scale and what weight i should be for my height, im obese. I dont know. I know weight plays a big roll in IF and i know they said just loosing 5% of your body weight can dramatically help you. I have been on WW since April and have not been as strict lately, but am trying to lose whatever I can before baby time. I have lost 5% of my body weight but nothing has happened yet. I didnt respond to my first round of Clomid. I am currently on Provera to end the cycle and will try a stronger dose of Clomid next. I know having PCOS sucks, I have it as well. Have you been tested to see if you are insulin resistant? If so, I would look into something to treat the IR (like met). I was put on BCP and told to lose weight as well. I have had the hardest time losing weight and I think it is because I am IR. I finally decided to see an endo whose specialty is PCOS to hopefully treat the PCOS rather than just going on bcp.
Actually I have read 20% is supposed to make a difference, I wish it really was 5%, I might have already tried to lose the weight. I am obese, doesn't matter what my height is, unless I was maybe 7ft.

All hormones, glucose and insulin levels are normal. It's just the damn ovaries that show the signs. I will speak to him after this next cycle and see if I could just be on met or would bcp during weight loss would be a better option. I want to read up on the medication and other things more.
I know when I tried to lose weight during spring of 08, I lost 2 lbs a week no problem. I was doing Slim Fast (I know it's not the best) and working out maybe 3 times a week, nothing strenuous. So I assume losing weight wont be too hard for me.
