Questions about annovulation and progesterone oil shots

I am annovulatory, I am on a break because my cycle was messed up this month and I had no follies. (well I had 2 very small ones that shrunk and disappeared)

 Anyway, my RE thought I would likely get AF on my own around the regular time. I didn't, I'm on CD49. I have not gotten it on my own for years now.

I am going to take provera to bring on a period, but I guess my question is:
Does this mean I have low progesterone? On my next medicated cycle should I ask to do progesterone oil shots? If so, do you start from CD1 or from O (in my case trigger)?

  • You have little to no progesterone any cycle you're annovulatory. Progesterone comes from the luteal cyst that's left over from the ovulation. You can't get your period on time without the dying off of that cyst (and it only dies off if a fertilized egg hasn't yet implanted and signaled by releasing HCG).

    I do not think that means you need PIO (progest in oil shots). This just means they put the horse before the cart with your last protocol and should have made you bleed (at least break through) + given your ovaries a rest (BCP) or something before stimming you again. You likely NEED to shed some lining from your last cycle.

    The provera (another version of progesterone) will aid in this - its not the TAKING of it, its the taking and then STOPPING of it that brings on a period. THat's why it takes a few days to get it post provera.

    If I do stim, get follies and trigger them I shouldn't have low progesterone and need pio? RE said it couldn't hurt but I'm not sure I understand exactly what it does, and if I need it or not.

    Yes, you should produce some progesterone.

    Most REs supplement with some sort of progesterone in the LP b/c it's so important to implantation and endometrial development. 

    Have you never had your progesterone checked @7DPO? (Maybe not if you don't O, I suppose) If not, have them check it and maybe ask to be monitored?

    For some reason the 7dpo (or about cd21) test for progesterone has to be sent to the provincial lab and takes about a week or 2 to come back. I have had it tested 3 times. #1-before any treatments at initial visit, it was really low.  #2-a clomid cycle to determine that I really do not respond to Clomid and #3- A break cycle a few months ago when it was 5. I have never had it tested on a femara cycle. 

    I don't know if it is related, but I have lower estrogen and unless I take estrace with the femara my lining turns out very poor.
