We have one. We opened ours with our financial planner, so we didn't pick out the specific plan. I think ours is a Rhode Island plan, but I'm not positive. I saw a recent article online about the best states' plans, but I can't remember where.... Maybe google something about comparison between different states' 529s?
We did one through our state. There was a recent article about choosing one in Parents magazine last month or the month before that had some good pointers.

Carson Henry, born 39w, 2d, via emergency c/s due to no fetal movement and fetal distress. Seizures, IVH grade 2, brain injury, kidney and liver damage. Complete blood clot in the artery in his right arm. 27 days in the NICU. Now discharged from all specialists, excepts his kidney doctor, who will monitor him indefinitely. My tough little cookie.
We have one through the state. It's great, especially for great-grandparents. We were able to send each a contribution book so they don't have to go through us and they love the tax deduction.

We have one (it's already lost $500...awesome)...we used www.savingforcollege.com to compare plans. and if you google you can see some other "top 5" type lists. It's pretty overwhelming. We ended up going with a USAA plan because we have several accounts with them and trust them.
We set one up ourselves. It was very simple. I chose the state we live in since there are state tax deductions for the contributions you make. I researched the plan a lot also through the online websites. Yes, it does help that I am a CPA : )
