Yes, your doctor can refuse. It is elective, there are doctors who don't do it at all and there are doctors who will not do it under certain circumstances (i.e. before 40 weeks, if you are GBS+, etc).
In my experience there was NO pain. There was some pressure and it wasn't all together comfortable, but it didn't hurt me at all. However, even my midwife said that she had women crawling off the table backwards who she was more gentle with, so I may just be an anomaly.
I'm not sure if you can FORCE your Dr. to do it. I had it done last time and my Dr. wouldn't do it before I was 2cm dilated. My SIL just asked her dr. to do it last week (she was 1 day over due) and he said no (she was 1.5cm dilated and 30% effaced). She was pretty annoyed but doesn't have a good relationship with her dr. anyway.
I think you could definitely push for it.
I did have it done last time and it was pretty uncomfortable. I wouldn't say it hurt, but it was uncomfortable enough that I was wincing during the process and kind of holding my breath. It was over quick though, maybe 5 or 8 seconds or so.
I know Dr's who won't do it. It is almost always your choice to have it done. You have to have dilated in order to have it done. It also will not start labor unless you are ready. If it is going to work you will be in labor in 24-48 hrs. Had it done before and it was uncomfortable, but didn't hurt.
My doctor won't do it unless the baby's head is "firmly" in my pelvis, whatever that means! I'm 2cm and 80% effaced. And when I go in for my next appointment- I'll be 40 weeks, 5 days...and she still won't do it unless baby is "firm"!
I am 40w 1d and my doctor did it today. He said I was between 2-3 cm dialated. It felt different, very uncomfortable. But it was over in less than two minutes.

my ob is doing it in a week for me. and she told me it might feel like a cramp/contraction. gl!
Yes, it is your choice. I asked my doctor about it. My doctor said they like you to be a little dilated and obviously don't want to do it too early.
It was pretty uncomfortable. It was like a long internal check. I had it done twice and it hasn't worked for me.
I had it done. I was 3cm and 90% effaced. My MW said she would not do it before I was 38 weeks, 2cm, and 50% effaced.
I can't say whether it cause me to go into labor or not, but 48 hours later we were leaving for the hospital and had LO that night.
