Yes, I realize this is a natural birth board! But since I am facing a 42 week induction tomorrow night, I'd like to make sure this part of my birth plan is well covered (when I initially wrote it, I included these sections as an after thought never really thinking it would come to that! ha!)

Labor Augmentation/Induction
? I would like to try prostaglandin gel before pitocin is attempted

Complications / Cesarean Section Delivery
? I feel very strongly that I would like to avoid a cesarean delivery. However, if a cesarean is necessary, I expect to be fully informed of all procedures and actively participate in decision-making, as well as have the surgery explained to me as it happens.
? I would like Father, xxx, and Doula, xxx, to be present during the surgery.
? I would like our doula, xxx, to take still photographs and/or video during labor and the birth.
? I would prefer epidural anesthesia, if possible, in order to remain conscious through the delivery.
? If conditions permit, I would like to be the first to hold our child after the delivery, as well as breastfeed immediately after birth. Or to have the child given to father, xxx, immediately after the birth.

Hey, I'm in Dallas, too! Where are you delivering, if you don't mind me asking? And who is your doula? (Feel free to PM if you prefer.)
Anyway. Have you talked over the induction stuff with your OB/midwife? Using prostaglandin gel first and starting Pitocin on the lowest dose is going to be his/her call. So if s/he is onboard, you shouldn't even need to put that in your birth plan, and if s/he is NOT onboard, putting it in your birth plan isn't going to make a bit of difference. Same goes for all the c-section stuff -- it might be fine, but it might also be totally against policy.
Also, you might want to talk about ramping up the Pitocin slowly if your body does not take over with natural labor on its own (or if your OB/midwife is unwilling to turn off the Pit). It's fine if you have to keep getting it, you just don't want them cranking it up like crazy if you're already in a good contraction pattern.

I'm delivering with a hospital midwife at Baylor, and my doula is Delilah Ray (www.cherishedbirth.com). I was a little hesitant to go the home/birth center route - so the hospital midwife at Baylor seemed like a great alternative. They are a very natural friendly hospital and most of their standard procedures are what i would ask for anyways.
I have discussed the induction with my MW and the plan is already to start on the prostaglandin gel then slowly with the Pit. Really this is just so anyone else (nurses, on call OBs etc) know the plan too. Or if my MW slips on the sidewalk and sprains her ankle as she's leaving for the hospital that morning and I end up with someone random.
We haven't talked c/s details since she obviously wouldn't be the one to do it. I forwarded what I had to my doula and added in a line about double surturing for a later VBAC attempt, not having my hands restrained and being able to hold the baby "as soon as possible" after the birth since "immediately" won't be an option (nurses have to check him out first to make sure he's breathing, different from a vaginal delivery). But she assured me this hospital is the most "keep mother and baby together" hospital in Dallas.
Glad to hear your 42-wk induction was a good experience! Obviously I'm hoping and praying for the same!
my suggestions (induced at 41.5 weeks):
- IV v heplock?
- lights/music in your birthing room?
- Should the nurses suggest the epi for you, or do you want them to not mention it until you give them the OK (my nurse made me tell her twice that is what I wanted so she was certain)?
- Do you want to know your progress after internals (this might discourage you if your nurse says "you're only X cm")?
- any postpartum requests if you don't have a c/s: catching the baby, rooming in, eye cream, pacifiers, formula/water, etc.
My c/s preferences were pretty much the same as yours. When I went over our plan with my OB, she made the suggestion of getting a spinal instead of an epi, because an epi takes so much longer to work. You may want to look at the differences and see what you think.
No adivce from me. Just GL and hope that everything goes well for you! There are definitely drug-free induction stories out there, so I hope yours will be one of them!
Should I end up with a repeat c/s, my preferences include:
- Spinal vs. epidural
- Spinal placed before the catheter
- My doula & DH to be allowed in the OR
- Photography allowed (this one is a long shot. we were not allowed last time)
- At least one hand to be free (another long shot, but worth asking)
- The curtain to be lowered when DD #2 is born
- Skin to skin ASAP
- Establishment of BFing ASAP
- No pain management medications other than Toradol & Motrin to be administered
- Incision to be closed with internal double stitching, glue & steri-strips externally
Oh, you are in good hands! I saw Cecily (and Jane, before she left) with DS, up until I switched to homebirth at 36 weeks, and I adored them! Probably the hardest part about switching to homebirth was having to leave them. And from everything I've heard, Delilah is awesome too.
Given that, I wouldn't stress too too much about the little details. Your preferences sound pretty standard under the circumstances. Baylor and Cecily will probably do the right thing, and Delilah will get them back in line if they don't
