statement of parental consent - what does this mean?

M is doing a pre-K program provided by the school district in the fall. Part of the enrollment was a health assessment form that her pedi filled out. At the bottom, there's a statement of parental consent that says:

In order to better serve the health needs of my child, I hereby give my permission for the transfer of health screening records to school and other appopriate health professionals.

And then I had to sign and date. Is this just a formality?

  • I had to do the same thing for both kids to enter pre-K.  I'm pretty sure its just a formality and its giving the school permission to look at her assessment from the ped.

  • It's so they can get her vaccination records, etc if they need them.  Without that, the dr office can't release that assessment to the school.
  • Our parents have to sign that consent form in order for the school nurse to look over the childrens health records.

  • I THINK that means that if your child is hurt and has to go to the ER or something, that someone from the school is authorized to see her medical records to facilitate treatment if you aren't there.

    But I would definitely ask.

    And I would NOT allow the school to have medical records that I don't personally give them except in emergency situations.

    I know a few people who have no clue where their vax records are - some people don't keep them for some dumb reason.  Sometimes it's just easier to get them faxed from the doc if the parents are being uncooperative, too.  We work with some doozy patients, I imagine they're just as friendly with the schools as they are with us LOL

    Man, it would really piss me off if the school was requesting medical records from my child's doctor.

    I remember a few years ago that a bunch of kids in DC were kicked out of school because their parents would not provide their shot records. If parents are too stupid/lazy to do what they are required to do to enroll their children in school, social services should step in, IMO. The government (i.e. the public schools) should not just be granted access to personal medical records.

  • For this enrollment I did provide a copy of her vax records. What's interesting to me is that it's nothing official. It's the card the hospital provided when she was born and I've filled it out after ea. appt.

    I agree on not wanting them to have access to it all, it's really NOT something I'm comfortable with, for me or the kids.  I'm going to have a hard time with it when the time comes, I know.  But then, I have the shot records filed and in theory they won't need anything else, but on the other hand.. gah.

    We have to give DS's preschool/daycare a shot record signed by his doctor each year.
