1. How much does your 4 month old eat and how often?
DD is eating 6 oz every 3 to 4 oz when I give her a bottle of BM.
3. When did you start rice cereal? At 4 months? Or did you wait until LO had their Dr. apt?
I think DD is ready! She will be 4 months on Sunday (Father's Day). Not sure if I will start then or wait until her Dr. apt.

Mine gets a 5 ounce bottle every 2-3 hours. He rarely finishes the whole thing.

My guy eats about 6 oz of formula every 3 or 4 hours. We tried the rice cereal about 3 weeks ago, just after 4 months old. If we had kept trying, I'm sure he would be doing alright with it, but after the first time I did a bit more research and decided to wait.
My guy is in the 50th-75th percentile for weight already, so he is doing just fine on the formula we have been giving him. I figure I'll try again in another week or two.

4oz of BM every 3 hours when not with me. When we are together I E-nurse him .. about every 3 hours also. DS is now on solids so he also gets about 5oz of pureed food per day (2-2.5 oz at 2 of his feedings).
3. When did you start rice cereal? At 4 months? Or did you wait until LO had their Dr. apt? Dr. gave us the go ahead to start cereal a couple of weeks before his 4 month appointment. Cereal did not go well so we moved on to purees - a hit.
My LO eats 6.5 ounces of bm/formula every 3-4 hours. I BF him for two of his feedings.
I haven't started solids yet and after our visit to the pedi today, I think I'm going to wait until 6 months. The pedi said 4 months is fine for solids if there is no family history of food allergies or asthma. Unfortunately DH had pretty bad asthma as a child, as did his sister and my brother. So we're waiting.
Eating 5-6 ounces every 3-6 hours for a total of 30 ounces per day.
No rice cereal even though like you said she appears ready (chews on spoon, grabs for adult food - and actually got a fistfull of some salsa, tongue pushing stuff out of the mouth has greatly diminished in the last month). We have had so many tummy troubles that we are not going to touch cereal until the GI gives us the go ahead and even then I am going to have him spell it out in writing on what to do and when and how.
I EBF from the boob (LO won't take a bottle anymore), so I have no idea how much she eats... but she eats around every 3-4 hours.
I'm going to wait to start cereal until around 5 and a half months or maybe 6 months. I've heard that it's best to continue to EBF until 6 months.
