Also - try sleeping w/ one of your LO's blankets for a few nights so it picks up your scent and send that to daycare teh first few days. Our DC suggested that for us and it really worked out well.
Send a shirt of yours and your DH. With my first she started DC at 6 mos, she and they loved having a picture album for her to see the pictures too. Make sure to mark everything.
Just noticed our LOs are ALMOST the same age. I start back to work Monday and take LO in then. I took him last week for one day to see if he would take a bottle from him. I am going to try to take the diapers and such tomorrow too, but I am not sure I will get there.

Is it a center? They should provide you with a list of some sort.
If you feed formula do they provide it or do you?
What about linens? Do you need to bring a crib sheet, bumper, mobile, blankets? Our old DC we had to bring everything.
Pacifier if you use them.
Bottles and a binky; lovie if they allow it.
