High-Risk Pregnancy

Any Type 2s on this board?

Hi ladies!  Sorry to just jump into the fray without an introduction.  We are expecting our second little one in late January of next year.  I got my BFP on Wednesday and we're very excited.

I have type 2 diabetes and was diet/exercise controlled before I got pregnant with DS. I was on insulin during my pregnancy with him and I'm on insulin with this pregnancy as well.

I have some questions about insulin therapy that I'd love to get some feedback from other diabetic moms out there.  Not medical advice, of course.

  • Hi, i'm type 2 and on insulin, this is also my 2nd, my first i only had gestational but i have been type now for two years.  i'm a total nerd about diabetic info so ask away!

    I am also a type 2 after developing gd four years ago with dd that never got better.  I am currently on insulin (NPH and regular).  Welcome and ask away.

  • Yes Ma'am! I am a type 2 for many years. We just recently found out that we are pregnant and due in DEC 2010.  Its our first child and I am scared out of my mind!  My endo was able to get me on insulin fairly quickly but was having issues at night so he requested for me to start using a pump.  My insurance blows so he gave me his teaching pump til my insurance gives in.  I am so glad to know I am not alone out there... cuz right now I feel so alone.  My husband has been supportive but we are both super worried! so i suppose I am with the original poster.. is there any advice we should know from the 2nd time moms with diabetes....

    So when I was pregnant with DS, I was on Reg and NPH, twice a day.  Prior to becoming pregnant, I was diet/exercise controlled for several years.  Pregnancy being what it is, things changed for me re: diabetes and after DS was born, I was on a couple of oral meds and Byetta.

    So now I'm back on insulin (which I like because of the control I can achieve). My internist currently has me on Novolog only, so no basal insulin.  I take 10 units at every meal and then test 2 hours PP.  Depending on that number, I use a sliding scale to correct.

    This is my third day on this protocol and honestly, I'm feeling uncertain about what to expect.  With DS, the two insulin types brought my fasting and post meal levels down considerably very quickly after starting on the medication.  This go around, I'm having a hard time bringing my fasting levels to my target goal (<90).  Post meals are getting easier.

    I guess I just need to know if I need to be patient and give myself a little more time to adjust or really advocate to add NPH to my regimen.

    Also, for the ladies who are on pumps, did you ask to be on one or was it offered to you by your endo/OB/pernatologist?  If things go the way I'm expecting, I would really like to consider being on one instead of MDI.

    I know how you feel! It is scary and statistics and horror stories about diabetes and pregnancy do not make it any easier (hello! Steel Magnolias, anyone??? Confused )

    But the really good thing is that while complications occur, they don't occur often. Further more, getting on insulin and into tight control early (which you've done) makes a huge difference!   

    I was freaked out with my first pregnancy and being on insulin and having to test my blood sugar and all the appointments, but then I really started liking the extra care and attention me and the baby were getting. It was comforting.  Smile

    I'm type 2, on an insulin pump! I love my pump and I will never go back to 5 shots a day!!  If you need anything ask away. I don't know everything, but I have been on insulin for 4 years. So, I know a little.

  • my doctor had the opposite approach and started with the NPH first, at night and then in the morning, my #'s after dinner was still higher than what we liked so we added regular right before dinner, things are going pretting good on that regime.  I would talk to your doctor, it seems to me that long acting control is better then adjust the short acting as needed if you need more help.  if you are just doing the regular the doses could get pretty high pretty fast.  Is you doctor a diabetets specialist? (endocronologist) or just internal?

  • that makes me feel so much better MrsAJL!!! it has been a rough 2 months... between coming off myoral meds, and allergy medication, and then being put on to insulin and then pump... its been so much change for me its crazy. And of course my body is so mad at me about the no allergy medication... I can hardly breathe... cant wait til i lose lung capacity :) !

     My doctors were all over this the moment I told them! so i feel like everyone is on myside, but sometimes its like pulling teeth to get information.  Everything i read online about Diabetes was not very good.. I callled my Endo darn near crying.. he had me come back in and re explained everything... now when i dont see him every week. Him or his wife (she is a pump specialist and trainer) will call me check my BG and make sure im doing okay.... I dont think i could have found a better Dr on that front...

    Like I said Im just  glad I am not alone.... its a scarything...and I dont like being a newbie at anything.... so thanks to all ya'll for being here :)

You only take 10 units before dinner???  what if you have something with high carb like pasta or patatoes or pizza? and then use the sliding scale to correct 2 hours later??  I only say something because for the 4 weeks i was injecting  I had to calculate my carb intake, check my sugar and then calculate my units... then check my BG 2 hours later... my carb to unit ratio changed several times, but i still had decent control... I guess it helps when you have diabetics all around you ( my dad and 3 friends I work with)  maybe a Endo doctor thats willing to treat pregnancy is a better bet... I only say that because when i first found out about my diabetes, i was seeing an internist... my dad made me go see his endo and in 2 months my A1C dropped 3 points which was awesome!!! I stuck with him ever since...

    As for the pump he recommended it.... but i kinda shot him down becuase of $$ but my mom and MIL heard about it and darn near killed me because that was my reasoning lol... so they are helping pay for it.  When he called to check in on me a few weeks ago I told him i changed my mind, he had me come in the next day to meet the trainer (his wife) and that night I was on a training pump... still am til medtronics sends mine....my numbers have been way better with the pump! I recommend it !!!  Now I am not sure if I will come off it when I deliver......

    anyway.. its just my opinion... but im not a doctor....:)

    You've got it right.  I started on insulin on Friday (so 4 days ago.)  We're starting with 10 units of Novolog at each meal and then the sliding scale to correct.   I'm not very happy with this situation because it seems to "shot gun" to me.  As for eating high card meals; I haven't been.  I've limited myself to 15-30 grams at breakfast, 30-45 grams at lunch and dinner and if I need a snack, 15 grams at snack time. 

    I, too, am a member of many diabetics in my family (mom, dad, aunts, uncles, grandmother, and grandfather.)  Only my father is on an intense insulin protocol.  He's been a great source of information.

    I will be faxing my numbers to my internist on Tuesday and I think I'll just ask to be referred to an endocrinologist to manage me during this pregnancy.  It can't hurt as I'm sure I'll need an endo's help once this baby is born.

  • I have been type 2 for several years now.  I have PCOS and it caused me to develop type 2 diabetes during my late teen years. This is my first (hopefully to term) pregnancy after a few miscarriages.  Currently I am on a slightly increased dose of metformin (Glucofage) as my insulin resistance is high and my endo wanted to see if I could avoid insulin by lowing my insulin resistance, which did occur.  My A1C and Insulin levels are looking ok for now, but I have a few more weeks of pregnancy to go.

    whoa hunny... i think you need to see a endo...only 10 units is crazy! with no adjust ment.... just does not make since... IMHO. thats not enough carbs.... i got moved up from 60 a meal to 80 because of the pump..right now i am craving pasta like its going out of style.... but i do try to stay away... makes me all sad..... i willing to bet you are eatting lots of meat?? and some veggies??  i know its hard!!!  and makes you really really tired since you have no carbs for energy... there is a website calorieking.com that i use to look up carbs..i love it but am very surprised at the carbs in veggies! maybe it might help you... i dunno..... at least thats my opinion.... :)  let me know how it turns out!

    I had an appt. this morning with my internist. He agreed that a) I needed to be on a long-acting insulin in addition to Novolog and b) I need to be working with an endo.

    So I have new meds and a referral to an endocrinologist. Smile Yay!

    Sweet Cookies!! I'm glad.... you were makin me nervous... lol... Im happy that you will be able to get way better control with an endo! 

     

    More good news I got my pump in today so i am going for "formal" training thursday lol... not that I have not had the loaner pump for like a month now lol.... hopefully you can get on a pump if thats what you want..... it makes life so much easier... and your able to open your diet a tiny bit more...as long as your basals and carb ratios are correct.... :) at least thats what I have been told... heres to PBJ sandwiches :) mmmmmmParty!!!
