Day 5 - engorged but where is the supply?

I had my twins on Wed and have either been BF or pumping every 2-3 hrs since then, with only a few exceptions. They are also receiving formula supplements.

Yesterday I became extremely engorged - breasts are huge, hard, and painful - but still only getting maybe 10 cc per 15 min pumping session and little relief from the engorgement.

 Is this normal? How long until I feel some relief?

  • I think it's normal to only get a small amount in the beginning. Newborns' stomachs are so small! Plus, the pump is never as efficient as the baby.

    Your supply will regulate itself and you will feel less and less engorged. Just BF on demand as much as possible and your body will adjust to what your babies need.

    If the pump isn't doing the job, you may have to nurse more. Primary engorgement doesn't last that long but you do want to take care not to get any clogged ducts by not emptying the breasts sufficiently. It sounds like perhaps your pump just isn't cutting it - that does not mean your milk supply is bad, in fact it sounds like it's good.

    If your supply is really strong you might want to consider dropping the formula supplement and just nursing. I won't pretend to know anything about nursing twins but maybe a LC can help. :)

  • I have been putting hot water on a small bath towel, and then massaging my breasts with the bath towel before I pump.  It helps a little.  I know how you feel.  Right now, my left boob is producing more milk, and my right boob is hard as a rock and little more painful.  I am hoping it will work itself out.  I am only pumping right now since I had a preemie at 31 weeks.

  • The LC told me I wouldn't pump much in the beginning, but also that primary engorgement isn't all milk it's also increased blood supply and lymph fluid in the tissues, which you can't do anything about.  I found a steaming washcloth and breast massage before feeding (or in your case pumping) went a long way and I definitely took a lot of Advil those couple days.  GL!
