Infertility

need advice...cancel or not??

the RE's office just called with my test results. I have  on the right a 17, 16, 13, 13, 12, 11. on the left a cyst, 16, 14, 13 and 10. but my E2 is 3068 so they are worried about OHSS. They left it up to me to cancel or proceed with IUI this weekend. They are putting me on destonex either way. My insurance is only covering me for one more month so i really dont want to waste these meds and i have a good feeling about this month.

I told them i want to still do the insemination, did i make the wrong decision??

  • Wait...

You have 10 follies and they are leaving it up to YOU? That seems really high. I thought most REs wouldn't proceed with IUI with over 5 follies.

I would be terrified of HOM with that many and I would personally cancel and stay far far far away from my DH.

  • they are figuring that i will only have two good eggs since i always have trouble with my left ovary and tube. they are just automatically counting those out.
  • That's a pretty high e2 for an IUI - mine was around that for my IVF cycles! In my opinion, I think there are way too many follies to chance it.  A cancelled cycle sucks, but I think OHSS or HOM are worse!

  • Well, you are more than likely going to release 4 mature eggs, so you are @ risk for multiples. The 17 and 16's are definite, and I think with your E2 that high, that 14 is going to grow enough and the trigger will make it grow more. I've triggered with a 14 and had it mature, so I think you are looking at 4 eggs.  Your E2 is quite high. I've triggered at 3000 before and had moderate OHS. Very very sore for a few weeks.

We can't tell you what to do or what not to do. You just need to understand your risk of HOM and complications from OHSS.  I will tell you that I lost triplets (from TI) and it was the absolute worst part of my life. Nothing worse than having a few heartbeats and ending up with nothing. Multiples are dangerous to your health and the babies. Also, I supposedly only had 1 follie when I got pregnant with the triplets. Obviously she couldnt see or measure everything, because I had a TON of follies. This is probably the case with you, so you may want to take that into consideration. You could very well have 5+ mature eggs, and that, + and IUI, is just asking for trouble.

My answer now is much different than it would have been before my losses. Before, I would have definitely preceded, but knowing the pain of losing multiple babies now, I would not. But you have to do what you feel is right for you. Go with your heart and what will be will be.  Just know that you could be in for a very dangerous and heartbreaking road. I say this because WE are not the ones who will have to live with your decision or the consequences or the heartbreak that may result. You will. You will get judged a lot for this, and your RE will be questioned about this. Don't be surprised if you get called irresponsible either. This is because the IF world/treatments get such a bad rap, and people see this as not helping things (think Octo mom).  But really, you need to soul search, but understand these risks are very real.

Is converting to a TI cycle an option for you? While obviously I am a great example of how TI can and does result in HOM, maybe it would reduce your risk a little.  Since your insurance is about to run out, I understand you not wanting to cancel. But a lost cycle is better than what I have gone through, trust me. Is converting to IVF impossible?

    Whatever you decide, good luck.

  • Indifferent

Not a chance in hell I'd go through with an IUI or have any sort of intercourse (timed or not) with that many follies. I'd either cancel completely (and use protection) or convert to IVF.

I'm honestly floored that they are letting you make this decision...my RE automatically cancels any IUI cycle that has more than 2 follies (sometimes they'll move forward with 3 if there is MFI).

I would not count those out, I believe that is flawed reasoning. The sizes don't lie. Is your tube blocked?

  • I had 2 mature eggs and 1 about to be mature and got pregnant with triplets..

That many eggs I'd run away.. run run run.

I would cancel.  It's not worth risking HOM and OHSS (both very serious)
