You switch to level 2 when your LO starts getting frustrated with Level 1. Some signs that Tegan showed, moving her head back and forth while eating, getting frustrated and playing with the nipple while eating and she started to take FOREVER to finish a bottle. We switched to Level 2 and everything was much better.
Your child doesn't have to wear shoes until they are walking and even then they should only wear them if you are going to be out in public. Barefeet at home is best.
Re: shoes -- as PP said, not needed until LO is walking, and really only when out and about. At home, barefoot is best. Soft crib shoes are nice for the pre-walking stage, when LO is walking with your support, outside for instance, and you want something to protect his/her feet, like on concrete. But don't use a rigid-soled shoe until baby is walking independently.
ETA: And when you do buy shoes, flexible-soled shoes are best (like Pedipeds or Stride Rite).
