Have you tried 1 cycle of IVF before a myomectomy?

2 drs said I should have surgery before going forward with IVF. I had tried 4 cycles of IUI and failed. And now they want me to have a myomectomy to remove a 3 cm fibroid, 2 cm cyst and look for endometriosis. I already scheduled the surgery for June 3, but doubting the surgery. If money was not an object I would totally try 1 cycle of IVF, but everyone says that doing the surgery will maximize my potential of getting pregnant. I know these drs are going off of stats and experience, but does a 3 cm fibroid (so small) prevent me from getting pregnant? Maybe my issue is fertilization? Have any of you ladies tried IVF instead of doing the surgery first? If I can get pregnant without getting cut open (c-section incision) that'll be great. Please share some of your stories if you can.  I think I need a peace of mind. =)

  • I had a lap surgery done before my Ivf cycle. Im glad I did because it helped us realize whats really going on in there. I was dx with severe endo and they found a 3mm cyst and fibroids while in there. The plan was to start with IUI's but seeing the damage that was done from the endo, the scar tissue was all over, we have no choice but IVF. Im very happy that I decided to do the surgery because it saved me a lot of time and pain in the long run, and money too. We are completely OOP so the IUI's would not have been covered either.   • It is pointless to go ahead with IVF before clearing out your uterus and making it perfect for your embryos.  Even if your issue is fertilization, if the embryo doesn't have a perfect lining to attach to then you are still in the same place.  Get the surgery.

  • I'm torn in my response to this. I had my myomectomy in Sept. '09. While my dr. was in there, he found a tumor on my colon and they went in a week later to remove 6 inches of my colon. My colon surgery caused an abcess on one of my ovaries. I don't know if it's related, but now I am diagnosed with diminished ovarian reserve. I was told when I had the abcess that it could cause problems with my ovaries and there could be long-term effects.

    I am grateful to having the surgery, since I wouldn't have known about the colon tumor without it. I will always wonder, though, if that's the reason for our DOR diagnosis and we're still unsure if IVF will work.

    I'm sorry to be debby downer, and I don't know if my story will help. Keep in mind that my case is an unusual one, and most myomectomies are not as complicated as mine was. I just wanted to give you both sides.

  • The decision to have a myomectomy is a huge one.  It is not a minor surgery.  To answer your question, yes - a 3 cm fibroid can definitely prevent you from getting pregnant depending upon where and what type it is.  For example, if it is actually penetrating the inner part of your uterus, you could have implantation issues/difficulties.

    Luckily, mine were not pertruding into my uterus, they were in my lining.  However, they were effecting the shape of my uterus and there was no way DH swimmers could have ever made it or a baby could grow safely - even though Dh had MFI.

    If money were not object, I would get a second opinion (which it looks like you did).  I would also ask why specifically they feel it would maximize your options - is it because of the location or type?  Also, fibroids have been known to grow very fast with pregnancy.  That could be a reason why they are suggesting you have the surgery.

    It's a difficult decision to make.  But for me, it was the best option.

  • Thank you ladies for all of your input and experience you shared. The fibroid I have is submucosal, so I was told it is denting my uterus. I was also told that my ovarian reserve is depleting so I need to take action quick.  A myomectomy I know is not minor. I was hoping I can do it laparscopically which I went to a surgeon that does that and said he can totally do it, but the other 2 fert drs said that a myomectomy is better for me b/c they can feel around for any add'l fibroids I have that didn't show up on the u/s. ugh.

    Due to your ovarian reserve, can you go through an IVF med cycle (or two) and freeze any embryos?

  • I was told that I should freeze some embryos if they are of good quality and viable. The doctor said that sometimes when you have a depleting ovarian reserve, the quality may not be good. Yeah like that's not depressing news right.

  • I did one fresh IVF & 1 FET. We thought our only issue (ha only)) was MFI, but on my hysteroscopy pre-IVF my RE found a "fibroid" which was small, but he sadi he had never seen anything like it. The tissue was weird & flimsy. So I had an operative hysteroscopy then the 2 IVF's.

    After they both failed I was certain it was this fibrois causing problems. We did an u/s & it was still there. So I had the mysomectomy Jan 2010. the pathology came back as an "angiolipoma' which my RE said was it was publishable it was so rare. I did have to wait 3 months for my uterus to heal & did my IVF #2 this month. i am currently in the 2WW, beta is Thursday.

    So we will see if that was my problem before.

     Oh wow. That was alot to go through. But I'm glad you can share that with me and with all the girls responding, it gives me a peace of mind that what I'm doing (the myomectomy) is appropriate.  I guess I'm just nervous and don't want to get cut open. =)

    Hi there! I'm sorry for getting back to you so late. So yes, I did do 1 cycle of IVF in June and it didn't work. =(  I was really bummed. I had 4 embryos that fertilized, but I only put back 2 b/c my husband and I wasn't certain we be ok with selective reduction after trying so hard to conceive.  I know hindsight and what ifs, but I wished that I did put in all 4 to maximize my chances.  So now my option or rather my next step is to have the myomectomy. I do have a 3 cm submucosal fibroid and a 2.7cm cyst which the RE drained while he was retrieving my embryos, but hopefully he can remove the skin of it.  He said the myomectomy is the best option for me rather having a laparscopic surgery.  The myomectomy for me will be more thorough.  Have you had your myomectomy?
