Usually I get someone to hold/watch the baby for a second but I have paid a visit to the little girls room while babywearing before. You just make sure everything's tucked up and doesn't get in the toilet. Not much of a problem, especially with the Ergo.
I go to the bathroom while I'm wearing him in the Boba. We were just at Disney World and I did got some very strange looks when I would walk out of a stall still wearing him..lol.

I don't have an Ergo ... we currently have a sprout pouch and I just ordered a mai tei style carrier ... anyway, everytime I need to use the restroom and the little man is in the carrier I just do it while I'm wearing him and I work around the sling. He doesn't really seem to care/notice and he has severe separation anxiety (has since 4 months) so if I were to leave him with anyone but dad or the nanny he would totally freak out. It's just easier for us if I don't pee alone. LOL
I just go to the bathroom, and be mindful of straps/cloth/etc.
I was peeing the other day, and DD was very interested in the sound, so I just explained to her that Mommy was urinating, and that I couldn't go in a diaper like she could, I had to use a grown up potty. If she just heard the peanuts teacher, eh... she'll learn eventually!

I use the bathroom wearing the ergo. I have to say, compared to a ring sling or a mei tai, I think it's a little harder to use the bathroom while wearing an ergo in a front carry. Just because the front of the ergo comes down and covers my button and zipper so it's awkward to undo and redo my pants. But it's not impossible.
Re: question I have been meaning to ask...ergo...
I've gone to the bathroom while wearing K in the ergo, the peanut shell, the mei tai and our ring sling.
Just make sure you know where any extra fabric/straps are, and it's pretty easy.
