You don't have to unless you drive a manual. Of course, most people will probably say you should always, no matter what kind of car you have (manual or automatic) because you never know what the computer is going to make the transmission do, especially nowadays.

Mostly yes. (I drive a stick). However, I have driven a stick that needed a kickstart with the gas and since you HAVE to have the clutch in to start, I always had to have the emergency break on, gear in neutral, and do a simultaneous gas and clutch start up.
Occasionally I catch myself doing this in my 'stang. But I always use an emergency break in park so its not a biggie.
I drove only manual for 20 years (until this car).
I now actually either have my foot on the gas (DH teases me that I rev it as I turn it on - like his grandma), or on the floor.

This. I always have the E break on. And then I let the E break off, push the brake, and put the car in drive. But I don't push the brake to start the car.
