Please comment too - I'm trying to figure out how to reorganize. I separated upstairs and downstairs because the fact that we are two story I think it what has been holding me back from putting it all upstairs. [Poll]
I answered separate toy/game room, but it's really our formal dining area that we converted to a toy room after Christmas. It's right off our main living area, and visible (no doors), but there's no toys in our living room now.
When we were still in a 1 story house we'd managed to move most of the toys into O's room. Now that we're in a 2 story we dedicated the living room as the kid play area. The family room is where DH and I hang out to watch tv.
We were in a small 1-story house, and now an apartment (which is bigger than our house was!), so we didn't have the upstairs/downstairs issue. We tried to keep most of the kids' toys in their bedrooms, but for the ones that were in the LR, we used decorative baskets to hold them so they wouldn't be visible. Anything that was big and couldn't fit in the baskets would go back to their rooms when they were finished playing with them.
Currently, there's a big bin in the living room that is overflowing with toys and a toy box in K's room that is overflowing with toys.
I've been looking for a cute smaller rolling bin to go in the living room. Once I find one, toys will be cleaned out and taken to goodwill or something and I will regain control of the living room! K (and Sam) will be able to roll the bin to the living room for play, but once they are done it will go back to their bedrooms. I am so so so sosososooooooooooooo tired of having toys everywhere!
everything right now is chaos. Toys are mostly in her room, waiting for the playroom to be emptied of moving boxes, then they'll be moved into the playroom. We have a small selection of toys and books in the living room.
We turned our den into a play room so 99% of his toys are in there. There are a few things out in the living room, but there are no toys in his room. He really only sleeps in there.
Her playroom downstairs is basically our formal dining room (we never got any furniture so it was an empty room). And some toys are in the family room. Her bedroom is upstairs but we don't really keep her toys in there yet (except for all the stuffed animals/dolls). We have a loft upstairs that I eventually want to use as a big playroom so we can reduce the clutter downstairs.
I put that they have taken over. His bedroom is upstairs and has a few things that can entertain him while we are up there...but mostly just his books. The main area for his toys is in the main living space but currently he has a play slide in the formal dining room b/c we don't have a dining room set at the moment.
I voted other. The majority of T's toys are in his room but we keep some in the living room. We have a one story house so it makes it easier to only keep a few in the living room. I try and rotate them pretty often.
My cousin has a two story house and lets the kids choose which toys they want out that week (in the living room) and the rest stay in their room. Every week they bring new ones into the living room and put the "old" ones away. I really like this idea because I have a fear of the toys taking over my house. Like, obessive can't stand too much clutter kind of fear. (not saying I think toys in the living room looks horrible, just saying that I like to keep it to a minimum at my house ) Once T gets older I hope to do the "choose this week's toys" thing with him.

I think age plays a huge part in this too. Even 6mo ago DD played in her room a lot less than she does now. She wanted to be closer to me so ALL the toys used to be in the living room, but more and more they are slowly moving to her room.
I think the key is to get some non-kiddie looking storage.
We started out with an open toy basket from ikea but stuff got lost in the bottom. Now that basket only holds stuffed animals (it's about 2'x1'x1'). We got some Expedit shelves that have 8 cubbies each. The bottom 6 on each hold toys and the top 2 hold our stuff in baskets. Some of the toys are in bins, some are larger toys that sit in the cubbies. There's another one in DD's room laying on its side so she has 8 cubbies there she can access.
The cubbies really helped me feel better about the look of our living room despite the fact that her huge pink play kitchen is still in the living room too.
They are all in her room - we are in a small one story. She is almost 6 though, so it makes it easier, obviously, because she can take them all there and clean up herself.
When she was younger and we were in a 2 story, the majority were in her room but we had an ottoman downstairs that we used as a toy box and had some toys in a storage closet down there too.
We also have a 2 story with all bedrooms upstairs. Now we are at the point that all toys are upstairs in their rooms or the gameroom. Last year when they were younger we would keep a tubberware bin downstairs with a group of their toys in it. With the house on the market we have to keep the toys contained but I really like having them all upstairs and the kids know exactly where they go (no more tubberware bins just thrown in). Of course they are older and can do this ) They will bring things downstairs to play with while I am cooking or whatever but they know they need to take it back upstairs when we go up for bed. This last weekend I actually had to get a trashbag and start throwing away to show DS I was serious - he got them upstairs quick!

We have a "formal" dining room right off of our front door (the horror that we keep toys in there, I know!), and since we never used our dining room, we moved the furniture out and now it's her dedicated playroom. Everything is in there. No toys are in her room except for a couple of stuffed animals in a wall-hanging froggy thing. I am a STICKLER about what toys come into my house, and more importantly, stay in my house, so we don't have enough toys to take over the house because I would freak out if that happened, LOL! It's the organizer in me, I suppose...I am a bare minimum type of person, and that applies to toys. I mean, she is only 2.
In her playroom is a thing from Ikea that hangs from the ceiling that has her stuffed animals and some dress-up stuff, her play kitchen, a 9-cubby ClosetMaid storage system from Target with cloth bins which stores all of her toys (if it can't fit in there, we don't keep it or get rid of something else ~ everything MUST fit into that system except for the really big stuff), a small cloth play house, and her art desk. She also has a couple of other toys that have to stay out b/c they're too big, like her baby doll stroller and a ride-on ice cream truck. It's so nice to keep it clutter-free because then I don't have to worry about the toys taking over everything, she has a dedicated playroom downstairs where she can play and I can always see her, and ALL of her toys go into one place. It's awesome; I highly recommend it (I have pictures of her room in my blog if you want to look; link in signature).
Once she gets older and plays upstairs more, we'll probably use the bottom room as a dining room again, but that won't be for a long time. I LOVE having her playroom downstairs.
