  What does she like? I ask because my MIL likes NOTHING.  Seriously, she hardly leaves the house, doesn't wear makeup or jewelry, hates to be around people (other than family) so I can never think of anything to get her.

  Ideas I'm throwing around for my mom:  Massage GC, GC to Sephora, a DVD set of a TV show she likes, tanning package, or mani/pedi GC.  Not sure what I'm going to settle on tho!

  We usually send food and flowers - something from Dean and Deluca usually. They live in MI so we have also sent a dinner for 2 from Legal Sea Foods (live lobster, clam chowder, etc.) and that was a hit.

  This year I think we are buying her a webcam so we can skype.

  Does she like jewelry?  Check out etsy for something that is ready to ship (like a grandmother pendant or something).  

  Membership to a museum?  GC to a Organic Delivery service?

  You can get things personalized while you wait at Things Remembered...so a jewelry box or something. 

  Maybe just get some pics of LOs nicely matted/framed.

  Food or flowers...things that don't "last" are good gifts for people who have everything.  My mom didn't have everything but she had very little space to put "stuff".  She only liked certain perfume (and had plenty of it), didn't wear much jewelry, had plenty of clothes, and was diebetic.  I always got her flowers and she loved them.  Once they died they just got thrown out and that was the end of it.
