If I was bottle feeding I would probably have done it by now. But since I'm nursing and she is still so inconsistant with it....we only go for walks by ourselves. There's just no way I can do it alone.
We started doing it immediately. I knew if I waited I'd build it up in my mind and would be much more afraid later.
We started with really quick trips, to Target or something like that. I'd go right after Charlie ate, so I knew I wouldn't have to feed him - we'd be gone maybe 30-45 min. Do you have a playgroup? That was another good place to take them both.
I pretty much started right from the beginning. You wont get good at it until you do it a few times. I was nervous at first, but soon it was normal again. I take them everywhere. We go to the grocery store or any shopping, I take DS1 to the TotPlex to play (and used to wear DS2, but now he can kind of play too), take them both to DS1 tumbling class.
We started as soon as I got the OK to drive after my c/s (2 weeks). Ditto Rach about building it up in my head otherwise.
We just got our double stroller, so we haven't been anywhere that I didn't have a grocery cart (DD is so not good about holding hands/staying with me) yet, but we will start doing those trips (no carts) this week.
It's really not as big of a deal as I feared it would be. Once you figure out your system, it's pretty simple, actually
3 weeks when my mom went home. We go to Target, grocery store, playground, playdates, walks around the neighborhood, etc. Some outings go better than others but so far it hasn't been too bad.
2 weeks or so. If I let my toddler get bored, it's going to be a long day, so we keep busy with errands, play dates, story time, playgrounds, picnics, etc.

I took them both to Babies R Us when ds#2 was one week old. I felt like if I didn't just go for it and do it so that I could see it was totally manageable I would probably still be sitting in my house!
Besides the Pedi's office, for about 2 weeks. It would have been sooner if we weren't in the middle of flu/RSV season (DD#2 is a preemie).
Since we were taken off "house arrest" we have been everywhere. I nurse DD#2 at the park or wherever I need. DD#1 is pretty chill and doesn't mind to sit in her stroller.
about 3 weeks. I had a csection & that's when dh went back to work. We went to the grocery store. I have taken ds2 with us to story time, playgroups, & mygym. I have a sling & an ergo.
