ETA: I'm pretty sure that it was covered under our ins, but not 100%. We seem to have pretty liberal insurance when it comes to things like lab work, u/s, etc.
My RE did ours, so I can't really give a recommendation. Just make sure you go to a clinic that does it regularly, not some place like LabCorp or somesuch.
Insurance didn't cover it (since we have NO fertility benefits at all, not even diagnostics!), but it was just 100 bucks out of pocket.

I was going to recommend him. He really did know his stuff. Ours would have been covered but for some reason we paid OOP. It was 100 dollars.
No idea what the doctor's name was, but he was in that red buidling next to Womens - you know, the one with the statue of the women tossing the baby in front of it. (does that count as a recc? hmmm, maybe not). They had a "special room" for collecting samples with reading materials and DVDs.
Anway, it was $125 and insurance did not cover it.
