I've had Zoloft for two weeks now (1 week 50 mg, then 1 week 100mg) and I'm not at all feeling better, it's actually gotten worse. I've had some thoughts related to suicide but not actually doing it - just wondering if I would be missed/would anyone notice kind of thoughts. And also about just punching a wall or something hard until the physical pain takes over all the other emotional/mental pain. It's really scaring me and I know I need to tell someone but not who, the doctor or the counselor? I'm supposed to call the OB tomorrow about how I'm feeling. Should I tell him about the horrible thoughts or just that I'm not feeling better? He mentioned trying something else if the Zoloft hasn't started to make any changes within 2 weeks. I also have a counselor that I see once a week so I could also go talk to her about the thoughts instead of the doctor. What do you think? Doctor or counselor?

Tell both and be 100% honest. You may also find it best to be under the care of a psychiatrist in terms of monitoring your meds vs. an OB. I believe in seeing a specialist for this stuff.
It can take longer than 2 weeks for the Zoloft to work, but anytime you feel worse or start having suicidal thoughts (and I'd call what you're experiencing suicidal thoughts), call your care providers ASAP.
I would call both.
Are you seeing your OB for your meds or a psychiatrist? If your OB is prescribing your meds, I would suggest considering seeing a psychiatrist since that is their specialty.
I am sorry that you are feeling worse. Not every med is for everyone. Hopefully things will get better on Zoloft or you will be prescribed something different. It will get better. It sounds like you are doing the right things--which is frustrating to be feeling worse. Hang in there and we are here if you need us!
