ABSOLUTELY NOT!!! Babies need calories to know when they are full, water doesn't have any calories, therefore they will not know when they are full and could cause water toxicity (don't remember the medical term, but means too much water in the system) and it can be very dangerous.

Every once and a while. Not really enough to matter though. Sometimes when she has hiccups and when she won't poop for a few days, I water down some baby apple/prune, although sometimes she gets the juice straight.

LOs Dr actually told us to give him water from birth but it is the one bit of Dr advice I chose to ignore. Eh...I think he gets everything he needs from BF.
When DD started solids last week we started giving her a sippy cup with Brita water. She drinks enough to flush out her mouth, then just chews on the sippy. No, they don't need water and yes, too much will throw off electrolyte balances, but an ounce is not going to hurt your baby.
I mix his cereal with water. I try to remember to get it from the fridge so that it is filtered, but sometimes I just grab it out of the tap. I zap it for 20 seconds in the microwave just to warm it up a bit. Since I've started mixing his cereal thicker, though, he's probably only getting 2T of it.
