Back From WTF Apt.

I finally got my wtf appointment and didnt learn much but we do have good news. We will go ahead for same protocol: clomid,pregnyl, and iui. He said it was great we were able to conceive and he thinks we were just "unlucky" and part of that 20% that m/c. He wasnt concerned about my labs that came back positive for lupus anticoagulant which scares me a little. He *thinks* it was because it was drawn after my d&c. They are not doing any repeat b/w or cd3 b/w either. He also said he wont start investigating until our 4th loss... This is our second and honestly I cant imagine anymore! Anyways, I am on cd18 today so within the next few weeks I will be back on the hormonal bandwagon. Woohoo!

  • What all did he draw on you? Did you have the complete work-up that tests for all the clotting factors? If not, I would not accept that. 4 losses and he will investigate further?  that is unacceptable. Do you know what your insurance requires? Do they have a certain # before you can be tested and it be covered?  What does he mean when he says start investigating? (what kinds of tests).

  • I am not sure what he means exactly. I had protein c, LA and not sure what else. My OB just ordered a few blood tests after because I told him it was our 2nd loss and we have a history of loss on both sides of our family. I thought he would atleast repeat the tests that were drawn 6 weeks ago to make sure this is not why I am having recurrent losses. Yeah, he said 4 m/c is "normal" and thats how they do it there. And there are no other RE's in the state.

    I agree, and I think you feel the same way from your post, and you should. Can you go to a clotting specialist/immune specialist for a 2nd opinion about lupus anticoagulant?  I would definitely do that.

    And I would probably look for a 2nd opinion with another RE too.  You really don't want to go through 4 losses before he gets more aggressive.  Sounds like he needs to be more aggressive now.  And at least double check your lupus test- I mean fine, it could be d/t the fact that it was drawn after your d&c,  but the key word is COULD.  He needs to verify that.

  • Yea I would totally find another Dr. That is total BS. There is no way I would have gone through more than two losses without testing. If you really can't get another doctor, I think you need to insist.  It shouldn't be up to him, just because he "doesn't do it". You/your insurance are paying for it. No way would I stand for that. If you have relatives (like your mom, sisters, etc) who have a history of repeated m/cs, there is a big possibility you have inherited a thrombophilia or an autoimmune issue. And although treatment sometimes doesn't help (like in my case) most of the time it can and does. There is just no reason to keep having losses when there is a possibility of a fix. And an easy one (like blood thinners) at that. You need to be tested for Factor V Leiden, MTHFR, etc. Someone posted a total list once. I'll see if I can find it. I would also want karyotyping for both you and DH.

  • I agree with pp'ers cutie and would ask for more testing or find another RE.  If you could prevent future heartache I think it would be worth the testing.  At the very least call insurance and see when they would cover additional testing.

    • Anti-phospholipid Antibodies
    • Anti-cardiolipin Antibodies
    • Lupus Anticoagulant 
    • Anti-Nuclear Antibodies
    Anti-Thyroid AntibodiesThyroid panel (TSH) Karyotyping of Parents

    that's utter crap.   I'd  be RE shopping if he won't at least  order an  RPL panel now.
