Hi ladies! DH and I decided not to do the biopsy and go straight to IVF with backup DS. Well the last couple of days we have been thinking maybe it would be best to do MESE/TESE before IVF to see if there is any sperm. If so, then we can freeze them, but if there isn't any sperm then we can go for IUI with DS and not have to do IVF. I was thinking this would be the best thing until I started reading that the chance of sperm surviving the thawing process is very low. I just keep thinking about how we might pay all the money for IVF and DH might not have any sperm and we are going to have to use DS, which we could of saved money with IUI. I hope this makes sense. I have so many thoughts and am not exactly sure how to explain it. We just don't know what to do....do we go straight into IVF with backup DS....or do we do MESE/TESE and freeze what sperm DH has or if DH doesn't have sperm then do IUI with DS. Opinions please...what would you do?

I would continue with the original plans an do IVF w/ DS back-up.
Sperm that needs to be retrieved is always better fresh than frozen and won't always make it threw a thaw.

I just wanted to add that I have no IF issues and have done 3 failed D-IUI's and am now doing IVF anyway.

It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

Hi! We did the TESE a few months before IVF so that we knew if there were sperm in there. We didn't want to go through choosing a donor and paying for DS if there were sperm to be found.
We used the frozen sperm for our first IVF and it was a BFN. Since we knew there were sperm, we did a fresh TESE on the day of my ER for IVF #2. I got a BFP today!
I've heard that fresh sperm really isn't that much better than frozen when it comes down to it. Even though fresh worked for us and frozen didn't, I believe that other factors got us our BFP this cycle.

FWIW, our MFI Uro pioneered mTESE and is considered one of the best Uro's in the country and he stressed to us that fresh mTESE sperm is always preferred to frozen.

How much is the TESE? Could you do it now and see if you have sperm and then do another if there is so that you have fresh sperm? We never went beyond the biopsy because DH didn't have sperm so the TESE costs are foreign to me.
If it were me I would want to know before doing IVF that there is sperm because it seems like a huge investment to turn around and use DS (if there aren't sperm) without trying D-IUIs first. That's just my opinion. Obviously you need to do what is right for you emotionally and financially.

I'd suggest asking both your urologist and RE what their thoughts are. Our urologist wanted to do MESA and TESE and have it frozen. We did have biopsy month before. Our RE said she definately agreed that was best for us and least stressful reguarding having to pick a donor etc. She found that there really weren't difference between fresh or frozen. Hopefully you'll get the same answer from RE and Uro and hopefully it'll make you feel better about deciding what's best for you. Best of luck with whatever you decide.
