we use one and have from hospital stay. he sucked his finger in utero and had a blister so to save him from more i insisted on a pacifier. we only use it at nap time and for falling asleep at night. typically once he is out i take it away. hasn't affected latch ever
I was nervous about nipple confusion so I didn't start until 2 wks when my son was really good about bf. i use the avent ones but I think each baby is different. I would get two different kinds to try at first. The paci is really a life savor- esp in the car. He hasnt had any trouble with nipple confusion.
we started using one pretty early- around 3-4 days old. like a pp, she sucked her hands in the womb and came out with blisters. she is an extremely strong breastfeeder so i wasn't that worried about nipple confusion...she still prefers the breast over binky anyway.
The NICU started them with DD to see if she was ready to start nursing and she's been an addict since. Unfortunately, I have to special order the pacifiers (GumDrop) online
I've used them since about 2 days old. He has never had a problem with nipple confusion. He liked the Soothie NB when he was first born, but now he is more into the Tommee Tippee ones. He'll take a Soothie if I can't find/haven't cleaned the Tommee ones though.
I was told to wait a month and I am counting down the days! DS like to use my nipples as a comfort and they are killing me! One more week to go (although Im sure if I started him today it wouldn't make a diff)
DS started using a pacifier the first night he was home with us. We offered one to DD in the hospital and she wanted nothing to do with it and still doesn't.

and my nipples thank me! We started at two weeks but it took a while to find one he liked. Only Mam 0+ months. Don't buy a lot of one kind.
