Does anyone know if I'm supposed to have some sort of 1099 or w2 or something to show income from NYS disability? I had both that and AFLAC disability.. while on Maternity leave.. I have a w2 from my work that says non-taxable 3rd party disability payments. I am pretty sure that was my AFLAC (I have been paying into that, so I don't think its considered income). I remember it saying something on the disability statements about having to pay tax on part of them.. I have the statements SOMEWHERE, and I will dig them out, but i already did my taxes (not filed yet) and didn't see anywhere to put that kind of income since I didn't have a w2 or 1099 for it.

I got a W2 from Liberty Life, which is where I got my disability payments from. I also got leave pay directly from my employer, so that is just in w/ my W2 from them.
Did you get checks directly from NYS?

WHOA. So I just called them.. and the lady was a TOTAL B!TCH!. She said that i should have gotten it from my employer.. Maybe it was the other w2 that I had.. I wonder why yours was from the insurance company though?
I want to fly to where ever this woman is and punch her in the face though..
