do i have to pre-mix her formula or can i carry the water in her bottles and the powder separately? i checked the tsa website and it isn't specific. and if i do have to pre-mix how do i keep it cold so that it doesn't go bad under the "one hour" rule?
also, do i have to carry some form of identification for her, i.e. her birth certificate?

We didn't have to carry ID for our DD when we flew in October.
She was on whole milk then, so we put it in a bottle and hoped for the best.
I would use the travel packets of formula, and either carry water in her bottle or carry an unopened water bottle. Maybe both. I know they tested the milk in her bottle when we flew, so they may just test the water in a bottle. Worst case, they make you dump water and you re-fill it by purchasing a bottle of water.
You don't have to premix it but I recommend filling your bottle with water because who knows when you'll need to make the bottle and where you'll be (you might be stuck on the tarmark and unable to get access to water). At security, they'll check the contents of the bottle, whatever it is (even if it is water) and then you're done. They won't care about the powder for the formula.

I carried it separately and never had a problem.
I also never carried a bc and no one asked me for it. But I've heard to have one on hand "just in case" (apparently I'm a rebel, because I never bothered).
When DD was on formula I always had the water pre-meausured in the bottle and a separate container for the powder. It was never a problem. I thought for sure they would do extra screening on it- but they never did. And I flew at least six times with DD. Never had a problem with it.
I usually put them in the same bin as my shoes. But several times the TSA agent told me I could just leave them in my diaper bag.
