We started doing it around 9 months. We used to do breakfast and dinner and added lunch when she seemed like she was asking for more EBM or nursing during the day.
Between 9-10 month - it wasn't everyday at first. We just sort of followed his cues. We also did BLW though so we were just offering an extra meal but not doing purees.
We've been offering 3 "meals" since 6 months, but we're BLW. Since he's not much of a napper during the day, it's easier for me to have him join us for all three meals so I make sure I get to sit down and eat. Some days we'll skip a meal here or there depending on how much we're out and about, but if I'm eating a meal, he generally has something at his high chair.

We started 3 meals pretty much as soon as we started solids. Daycare gives him carrots and zucchini (they make it, steam it/mash it), so he was getting breakfast and dinner at home and then lunch at school 4 days a week. On the weekends he would often just eat two meals a day, but now (at 8-9 months) we always do 3 meals, and sometimes 3 meals and 1 snack.
Somewhere in the 9 -10 month range. We followed DS's cues and added it in when he really seemed to be enjoying eat solids. It was hit and miss if he actually ate anything at all 3 meals for quite awhile though.

I know, right??? I have read... and reread all the info and I'm still like.... ?????? lol. How hard can this be???
12 months for us.
We added the second meal at 9 months.
Somewhere in between we added a snack of finger foods.
ETA: I wanted to make sure she nursed until at least 2 years, so we went very slowly with solids.
